This application must be filled in by typewriter or ink and submitted to the Building Department with the follmving:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and anusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plmnber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Plamfing Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

It has come to our attention that building permit #26438Z, for construction of an in-
ground swimming pool at 2600 Oregon Road, Mattituck, NY, Suffolk County Tax Map
Number 1000-100-4-5.3, has expired and you are in violation of Southold Town Code.
In order to rectify this matter, the following is required:

Immediate renewal of permit number 26438-Z: Please submit a
check for $150.00, made payable to the Town of Southold, noting the
original permit number on the check.
Inspection Required: Once you have renewed the above-mentioned
permit, it is your responsibility to schedule the required final
inspection. You will recall that you failed your final inspection, in
part, because the inspector felt that the fence you provided did not meet
NYS state code.
Certificate of Occupancy required for the expired permit: Once
you have passed the above referenced inspection, you may apply for a
certificate of occupancy.
Without a certificate of occupancy from this office, any use of your property is a
violation of Southold Town Code (45-15) and New York State law. If you have any
