Failur,~ to produce a copy of the permit upon r~uest by a Department representative is a violation of this permit. 2. Relationship of t~is.Permit to Other Department Orders and Determinations Unless expressly provided for by thc Dcpurtmcnt, issuance of this permit does not modify, supersede or rescind any order or determination previously issu~l by the Dc-.partmcnt or any of thc terms, conditions or requirements contained in such order or dcterrnination. 3. Applications For Permit Renewals, Modifications or Transfers The permittce must submit a sepm-atc written application to the Department for pe~,ifit nmewal, modification or transfer of this permit. Such application mu.st include any forms or supplemental information the Depa,'hl',ent requires. Any renewal, modification or transfer granted by tho Department must be in writing.
