This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic

I inspected the footings for the new structure pdor to being backfilled and found them to be
installed in accordance with the plans. I also inspected the ledger board connection to the existing
building structure and found it to be structurally acceptable.
To the best of my knowledge the completed work at the site is in accordance with the New York
State Building Code.

a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

General
* All work and matadale shall be in accordance with the New York State Building Code and Lccal Plumbing Cedes.
* All hedzontal waste and vent runs of 2 inch waste pipe are to pitched at Y4 inch per foot. The 3 and 4 inch waste
and vent piping should also have a 74 inch per foot pitch wherever possible with a minimum pitch of Y~ tach per thor.
* Provide all vents shown in the drawings, as required by the Plumbing Code, and as necessary to prevest sipbeuage
or back pressure on the trap seals.
* Provide cleanouts as shown in the drawings. Provide 18 inch clearance at all 1rap locations.
* All wasta and vent pipes, and water supply pipes are to be suppobed by pipe hangers, The hangers shall be installed
at a maximum spacing of 4 feet. All verflcel mos are to be supported as required by local and State Cedes.
* Pressure Test all waste and vent piping at the completion of rough in. Tightly close all openings in the piping system,
and fill with water to a point of overflow, but not less than 10 foot head of water. The water level is to remain level for a
minimum of 15 minutes to be considered leak free.
All water lines within the buildings are to be Type L copper tubing.
* All solder joints are to be made with lead free solder in accordance with local and State Codes.
* All water supply piping is to be insulated with t inch thick cylindrical molded glass fiber ~ybe insulation with a 3Y~ lb. bensgy.,
* Install ball valves at the locations shown on this drawing, Aisc installed gate valves for hot and cold water a{ all fixtures. All
exposed water supply piping to fixtures and the valves are to be 67% brass pipe chromium plated.
* Run water supply piping to fixtures concealed within walls and hedzontally to fixtures. All pipes are to be equipped with
chrome plated esehuteose at all exposed floor and wall penelm~ons.
* All water supply piping is to be pressure tested. The initial test is with no connection to the ~xtures and is to be done
by capping all openings and pressurizing the piping sys'[em with compressed air to a minimum pressure of 150 psig.
The pressure is to monitored for 2 beum and will be considered to be leak f~ee f/the pressure remains constant dedng
the test period. After successful completion of this test, the fixtures are ta be connected and the system is to be tested
at a pressure of 90 peig for a period of 2 houm.
* The water supply system shall be cleaned by filling the system with water/chlorine solution with at Feast 50 ppm of chlorine.
The piping is to isolated and allowed to stand for a pebed of 24 hours. The system is to be flushed at the comptation of the
test.
