This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Pursuant to the provisions of Chapter 275 and/or Chapter 111 of the Town Code of the Town of
Southold and in accordance with the Resolution of the Board of Trustees adopted at the meeting held on
July 23, 2008, and in consideration of application fee in the sum of $250.00 paid by Elizabeth & David Branch
and subject to the Terms and Conditions as stated in the Resolution, the Southold Town Board of Trustees
authorizes and permits the following:
Wetland Permit to construct an 881.44 square foot one-story addition to the
existing dwelling, a 177.42 square foot deck and screened porch addition onto an
existing deck, to be reconstructed, insta"]l~rywellsfand construct a new 4'x16' pad,
with the condition that the new walkway connected to the new pad is to be made of a
pervious material and in accordance with Chapter 236 of the Town Code-Storm Water
Runoff, and as depicted on the site plan prepared by Tour de Force Designs, LLC, last
dated August 19, 2008, and received on August 25, 2008.

Based on the information you have submitted, the Department of Environmental
Conservation (DEC) has determined that the property landward of the 10' elevation contour, as
shown on the survey prepared by Jacqueline Salame last revised on 12-09-08, is beyond Tidal
Wetlands Act (Article 25) jurisdiction. Therefore, in accordance with the current Tidal
Wetlands Land Use Regulations (6NYCRR Part 661) no permit is required.
Be advised, no construction, sedimentation, or disturbance of any kind may take
place seaward of the tidal wetlands jurisdictional boundary, as indicated above, without a
permit. It is your responsibility to ensure that all precautions are taken to prevent any
sedimentation or disturbance within Article 25 jurisdiction which may result from your project.
Such precautions may include maintaining adequate work area between the tidal wetland
jurisdictional boundary and your project (i.e. a 15'to 20~ wide construction area) or erecting a
temporary fence, barrier, or hale bay berm.
This letter shall remain valid unless site conditions change.
Please note that this letter does not relieve you of the responsibility of obtaining any
necessary permits or approvals from other agencies or local municipalities.

The building permits as noted above have been forwarded to us by John Bertani Builders. I do want to
point out something that is incorrect on the permits.
You are showing our mailing address as 166 New Norwalk Road, in New Canaan, Ct. That was a
temporary dwelling for us while repair was going on at our house at 681 Weed Street, and was changed on Southold
town records in 2005. As a matter of fact our tax bills come to our correct address from Southold, and our permit
application also showed a mailing (home) address as 681 Weed Street. I would appreciate it if you would change the
records you have in the building department in case you need to correspond with us. Thank you.
