INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building inspector wilh 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. Thc work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available lbr inspection throughout the work.
e. No building shall be occupied or used in whole or in part lbr any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
t'. Every building pemfit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting lhe
property have been enacted in the interiln, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

As you are aware a committee was formed to review the plans you submitted to expand
your home in our commtmity. The Committee members consisted of Edward Mclatyre,
Joseph Pfaff, and Salvatore Cioffalo. A meeting of that committee was held on 2/3/08.
At that meeting, I am happy to advise you, the plans attached to this correspondence were
reviewed and preliminarily approved by unanimous vote. We will be advising Mr. Olsen,
President of the Laurel Links Home Owners Association by separate correspondence of
this decision.
Please feel free to proceed with having formal plans drafted in relation to this project. It
is our opinion that the proposed preliminary plans are not in conflict with the current
covenants and restrictions in our community. Please submit your final formal plans for

Foundation Notss:
The contractor is to verify all measurements in the geld and any discrepancies are to be brought to the attention of the Engineer prior to conMmction.
1 - All concrete 3,500 psi after 28 days minimum.
2 - All rebar ASTM A-615 Grade 60.
3 - Footings are to be installed on undistsFeed virgin soil. The bottoms of all footings are to be installed a minimum of 3' below grade.
4- The Engineer is to be contacted if unacceptable or questionable soil is encountered during excavation. Unacceptable soil is soil containing clay and/or

Slab Sifo Preparation
* The area where the slab is to be installed is to be excavated to the necessary elevation to meet the final top of the slab elevation shown on the
Amhitectural drawings.
* The areas where the slabs are to be installed are to be mechanically compacted after backfilling of foundation walls in 6' lifts in accordance with ACl
302.1R. The Engineer is to be notified if unacceptable soils are encountered dudng excavation and if compacting produces meetis that exceed ACI
standards.
* The excavated areas along the stem walls and footings are to be back filled with clean matedal and mechanically compacted with clean fill in 6 inch lifts.
Each layer is to be compacted to 98% modified proctor.
* Base Matedal layer shown on the drawings is to be a compautable, easy to trim, granular fill that will remain stable and suppor~constmction traffic. A clean
fine-groined matedal with at least 10 to 30% of particles passing a No. 100 sieve but not containing silt, or organic mutedal is recommended.
Slab Notes:
* Isolation joints are to be installed between the slab and the walls. Use preformed joint gller that is to be cut Y~ inch below the slab surface and the resulting
joint is to be filled with an elastomegc joint sealant.
