This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with acCUrate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (8-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building~
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

During a review of our files it was noted that the above
building permit has expired, and a Certificate of Occupancy has
never been issued.
According to the Code of the Town of Southold, Article
XXVIII 100-284, it is unlawful to occupy or use a structure
until a Certificate of Occupancy has been issued.
Please contact this office as soon as possible so that we
may clear up this matter.

Dear Sir,
I am in receipt of your letter of December 14, 1995 regarding the
building permit which was issued for my residence at 2015
Stillwater Ave in Cutchogue. The work on that extension is
continuing but I am now trying to do most of the work myself and it
is taking longer than expected. I have discussed the work to be
done to completion with the electrical contractor and the
electrical inspector. I expect to be finished in the next six
months.

We are responding to your letter of January 22, 1996
regarding building permit #21483-Z.
Since your permit has expired as of 12/15/93, we cannot
grant you a six month extension. Your next course of action
would be to renew your building permit by sending us a check for
$75.00 payable to the Town of Southold. The new permit will be
good for 18 months, and if not completed by then you can request
in writing, a slx month extension giving you a total of 2 years.

Mr Gary Fish of your department completed the final inspection of my home at 2015 Stillwater
Ave in Cutchogue on 3/16/2011. Mr Fish stipulated that I must provide evidence of completion
of electrical certification and plumbing (solder) inspection. He also stated that I must provide a
drawing of the deck that has been included in this addition (see Final Inspection report
attached).
