This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey °f pr°perty with accurate location o f all buildings, property lines, strects, and unusual natural or

2. Fi~'al Appr°val from Health Dept. of water supply and sewerage-disposal (S_9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. S.wom statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Pl?ning Board Approval of completed site plan requirements.

sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premmes or public sti'eets

c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will i~sued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.

Dear Mr. Jarvis;
The following resolution was adopted by the Southold Town
Planning Board at a meeting held on Monday, May 24, 1993.
WHEREAS, Herodatus Damianos is the owner of the property known
and designated as Pindar Vineyards, SCTM# 1000-85-2-15, located
at Main Road in Peconic; and
WHEREAS, a formal application for the approval of this site plan
was submitted on March 5, 1993; and
WHEREAS, the Southold Town Planning Board, pursuant to the State
Environmental Quality Review Act, (Article 8), Part 617,
declared itself Lead Agency and issued a Negative Declaration on
May 3, 1993; and
WHEREAS, all the requirements of the Site Plan Regulations of
the Town of Southold have been met; and be it, therefore
RESOLVED, that the Southold Town Planning Board grant
conditional final approval on the surveys dated May 10, 1993,
and authorize the Chairman to endorse the final surveys subject

Pursuant to 144-15A, of the Southold Town Code, "No building hereafter erected
shall be used or occupied in whole or in part until a certificate of occupancy shall have
been issued by the Building Inspector."
Therefore, you have ten days fi.om the receipt of this letter to obtain your Certificate of
Occupancy as required, or your file will be referred to our Code Enforcement Officer and
the Town Attorney Office for legal action.
