Condon Engineering, P.C. on behalf of HERNAN MICHAEL OTANO requests a
Wetland Permit to replace the existing deteriorated and collapsing foundation and
installation of six (6) 8'X 2' deep drywells. Located: Main Rd. & Sage Rd., Southold.

The proposed action has been reviewed to Chapter 268, Waterfront Consistency
Review of the Town of Southold Town Code and the Local Waterfront Revitalization
Program (LWRP) Policy Standards. Based upon the information provided on the LWRP
Consistency Assessment Form submitted to this department, as well as the records
available to me, it is my recommendation that the proposed action is CONSISTENT
with the below Policy Standards and therefore is CONSISTENT with the LWRP.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in
preparing its written determination regarding the consistency of the proposed action.

INSTRUCTIONS
1.
LWRP CONSISTENCY ASSESSMENT FORM A ~ 2010
All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This ,
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actionx including BuiMing Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
Before answering the questions in Section C, the prepa~r of this form should review the exempt
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed action will be evaluated as to its significant
beneficial and adverse effects upon the coastal area (which includes all of Sontho, ld Town).
If any question in Section C on this form is answered "yes", then the propo~tqct~on may affect the

Foundation Notes'
The contractor is to redly all measurements in the field and
any discrepancies are to be brought to the Engineer prior to
constmcflon.
I - All concrete 3,500 psi after 28 days minimum.
2 - All rebar ASTM A-615 Grade 60.
3 - Footings are to be installed on undisturbed virgin soil
The bottoms of all footings are to be installed a minimum of
3' below grade unless indicated otherwise.
4 - Backfill around footings and pier is to be clean material
and is to be mechanically compacted in 6" lifts to 95% of
maximum dry density.
5 - The areas below the building are to be filled with clean
soil to grade level.
Framing Notes:
The contractor is to vedfy all measurements in the field and
any discrepancies are to be brought to the attention of the
Engineer prior to construction.
I - The 7 2/ by 7 ½ beams are to be ACQ No. 2 Southern
Yellow Pine.
2 - The 5.5 x 9.5 beams are to be Level 3 Wolmanized
Parallam by iLevel Trus Joist.
3 - All straps, connectors, bolts, nails, etc. are to be
galvanized, ZMax coated or stainless steel Designated
connectors, strap etc. on these drawings are made by
Simpson unless indicated otherwise. All connectors, straps
etc. are to be nailed/bolted in accordance with the
manufacturer's specifications.

EROSION CONTROL MEASURES
1, IN ACCORDANCE WITH THE PROVISIONS OF THE USEPA GENERAL PERMIT AND
THE NYSDEC, DURING THE COURSE OF CONSTRUCTION, CERTAIN EROSION AND
SEDIMENT CONTROL MEASURES MAY BECOME NECESSARY TO PREVENT THE
TRANSPORT OF SEDIMENT TO OFF-SITE AREAS, PONDS, WATER COURSES, DRAINAGE
INLETS, RECHARGE BASINS, ETC. ACTUAL EROSION CONTROL MEASURES WILL BE
DICTATED BY FIELD CONDITIONS AS CONSTRUCTION PROGRESSES, BUT THE
FOLLOWING GENERAL CONDITIONS SHALL BE OBSERVEB:
EXISTING VEGETATION TO REMAIN SHALL BE PROTECTED AND REMAIN
UNDISTURBED.
CLEARING AND GRADING SHALL BE SCHEDULED SO AS TO MINIMIZE THE SIZE
OF EXPOSED AREAS AND THE LENGTH OF TIME THAT AREAS ARE EXPOSED.
THE LENGTH AND STEEPNESS OF CLEARED SLOPES SHALL BE MINIMIZED TO
REDUCE RUNOFF VELOCITIES.
- SEDIMENT SHALL BE RETAINED ON THE SITE,
SPECIFIC METHODS AND MATERIALS EMPLOYED IN THE INSTALLATION AND
MAINTENANCE OF EROSION CONTROL MEASURES SHALL CONFORM TO THE NEW
YORK GUIDELINES FOR EROSION AND SEDIMENT CONTROL".
2. SEDIMENT BARRIERS (SILT FENCE, HAY BALES OR APPROVED EQUAL) SHALL
BE INSTALLED AS REQUIRED ALONG THE LIMITS OF DISTURBANCE FOR THE DURATION
OF THE WORK. NO SEDIMENT FROM THE SITE SHALL BE PERMITTED TO WASH ONTO
ADJACENT PROPERTIES, WETLANDS OR ROADS.
Scope of Work
* Sec,ute the services of a surveyor to vedfy the existing
eleva~lion of the grade around the building and the first fleer
of the building. Adjust the elevations shown on the drawing
as necessary to assure that the top of the finished floor is 2
feet above the Sase Floor Elevation at the conclusion of the
work.
*install erosion control systems as shown in the plan.
* Pump standing water from areas below the building into
the d~y wells
* Insball (6) 8' x 2' deep precast concrete dry wells in area of
lot sh~OWn in the site drawing on this page. Excavated soil is
to be, mixed with clean sand and used to backfill area under
buildgng
*lnstafl[ temporary shodng under existing building. The
buildi~ng is to be raised to an elevation at least I foot above
the fi~al elevation to facilitate installation of the foundation
syste~m.
* Remove existing brick foundation and other debds from
areas~ below the building. All matedal is to be removed from
the siite and properly disposed.
* Ex=avate soil as necessary to install new poured concrete
footings and piers. Excavated soil is to retained below the
buildi~ng as much as possible.
* Inst~alt new floor beams as shown in the plans.

* Set existing structure down onto new floor flaming system
and srscure all floor existing joists to the beams as indicated
in the~ drawing.
* Backfill the areas below the building to the existing grade
eleva~tion with clean sand mixed with soil excavated for the
dry w~ells and any matafia~ excavated in order to install the
feetimgs.
* Install aluminum gutters and leaders along the front and
rear roof edges. Install 3"~ drainage pipes underground
from the front and rear nodhem comers of the building to
the dry wells.
Regrade excavated areas and around building and plant
grass seed.
3. GRADED AND STRIPPED AREAS AND STOCKPILES SHALL BE KEPT STABILIZED
THROUGH THE USE OF TEMPORARY SEEDING AS REQUIRED. SEED MIXTURES SHALL
BE iN ACCORDANCE WITH SOIL CONSERVATION SERVICE RECOMMENDATIONS.
4. DRAINAGE INLETS INSTALLED AS PART OF THE PROJECT SHALL BE PROTECTED
FROM SEDIMENT BUILDUP THROUGH THE USE OF SEDIMENT BARRIERS, SEDIMENT
TRAPS, ETC. AS REQUIRED.
5. INSPECTION AND MAINTENANCE OF EROSION CONTROL MEASURES IS TO BE
PERFORMED DAILY BY THE CONTRACTOR PRIOR TO THE START OF CONSTRUCTION
FOR THE DAY AND AFTER HEAVY OR PROLONGED STORMS. MAINTENANCE
MEASURES INCLUDE, BUT ARE NOT LIMITED TO, CLEANING AND/OR REPAIR OF
SEDIMENT BASINS OR TRAPS, SEDIMENT BARRIERS, BERMSi DIVERSIONS AND INLET
PROTECTION.
6. APPROPRIATE MEANS SHALL BE USED TO CONTROL DUST DURING
CONSTRUCTION.
7. A STABILIZED CONSTRUCTION ENTRANCE SHALL BE MAINTAINED TO PREVENT
SOIL AND LOOSE DEBRIS FROM BEING TRACKED ONTO LOCAL ROADS. THE
CONSTRUCTION ENTRANCE SHALL BE MAINTAINED UNTIL THE SITE IS PERMANENTLY
STABILIZED.
8. SEDIMENT BARRIERS AND OTHER EROSION CONTROL MEASURES SHALL
REMAIN IN PLACE UNTIL UPLAND DISTURBED AREAS ARE PERMANENTLY STABILIZED.
AFTER PERMANENT STABIILIZATION, PAVED AREAS SHALL BE CLEANED AND THE
DRAINAGE SYSTEM CLEANED AND FLUSHED AS NECESSARY.
