This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of ail buildings, p?operty lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S_9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance fi.om architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing b.'uildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.

Dear Mr. Verity;
In conjunction with preparing the as-built drawings for the above referenced arbor style farm stand, I made
a physical assessment of the structure and found it to be not only sound but also well built with exception
to some cosmetic repairs which are to be made as part of this application for the building permit.
It appears by the age of the roofing that this arbor was built prior to the advent of the high wind connector
requirements. The nature of the construction is that of an open sided post and beam pole barn using 6x6
pressure treated posts set in the ground at six locations which support the 16' x 23' roofi There is no
documentation as to the existence of footings below the posts however in my opinion; the minimal load on
the posts relative to the soil conditions at the site as well as the nature and use of this open arbor would not
necessitate footings for uny additional structural bearing. This opinion is further verified by observing the
soundness of the structure as it exists with no signs of settlement or shifting over the years.
I trust this statement will act as a satisfactory certification as to the structural stability of this arbor as of
this date and look forward to receiving the C/O and farm stand permit as soon as possible.

c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
