THIS IS TO CERTIFY THAT THE FOLLOWING RESOLUTION NO. 2010-694 WAS
ADOPTED AT THE REGULAR MEETING OF THE SOUTHOLD TOWN BOARD ON
SEPTEMBER 7, 2010:
RESOLVED that the Town Board of the Town of Southold hereby accepts the proposal of L.
K. McLean Associates~ P.C. for professional services relative to the decommi.qsionin~, of the
scavenger waste facility~ Moores Lane~ Greenport~ NY in the amount of $15,200, all in
accordance with the Town Attorney.

Supervisor Scott A. Russell to sign a proposal for professional services in connection with
construction monitoring and administration duties, in reference to the decommissioning of the
scavenger waste facility, Moores Lane, Greenport, NY in the amount of $I 5,200, all in
accordance with the approval of the Town Attorney.

Re: Town of Southold - Scavenger Waste Plant (Greenport, Moores Lane)
Cost Proposal for Construction Monitoring and Administration Duties
Dear Mr. Richter:
L.K. McLean Associates, P.C. (LKMA) is pleased to submit a proposal for professional services
relative to the scope of work discussed with your office in our telephone conversation on August 24,
2010.
Based upon our conversation we understand our scope of services is to include the following;
Limited Construction Monitoring services relative to the decommissioning of the scavenger
waste facility along the three parcels defined in the contract drawings. Our office assumes
12 weeks of actual construction activity on the site and has assigned 10 hours/week for
inspection services.
Address any questions raised by the Contractor during the construction phase.
Process and approve payment request applications submitted by the Contractor and
coordinate the remaining payment process with the Town's Comptroller.
Review and approve any submittals/test results furnished by the Contractor during the
construction phase.
Schedule and administer construction progress meetings with the Contractor and Owner.
Address any concerns raised by the Owner and/or Village of Greenport.
Prepare inspection repons and obtain photographic evidence of the work performed during
the construction phase of the project.
Coordinate all mobilization and demobilization (project close-out) activities with Contractor
for final acceptance of project.
Our fee for the above referenced professional services is based upon our existing Annual
Professional Services Contracts with other local municipalities and past projects performed for the
Town of Southold.
Task I - Construction Inspection Services (120 hours total)
= $10,200.00
Task II - Construction Administrative Services (40 hours total)
= $ 5,000.00
Total Estimated Cost for Services
= $15,200.00
An appropriation of $15,200.00 would be required for the estimated engineering services associated
with the execution of all items identified in the scope of services. The appropriation would be a
cost not to exceed and shall include all out of pocket expenses associated with travel, materials
and reproduction costs. Any tasks not identified in this proposal or listed as "optional" shall be
considered unauthorized additional work. No additional work (surveying services) shall be
performed by our office without proper authorization from the Town of Southold.
Should this proposal be acceptable, please sign both copies of same and return one copy to this
office. This shall constitute our Notice to Proceed.
