Box 1179 Southold, New York 11971 TELEPHONE (516) 765-1801 THIS IS TO CERTIFY THAT THE FOLLOWING RESOLUTION WAS ADOPTED BY THE SOUTHOLD TOWN BOARD AT A REGULAR MEETING HELD ON JUNE 14, 1988: RESOLVED that the Town Board of the Town of Southold hereby authorizes and directs Supervisor Francis J. Murphy to execute a Maintenance and Inspection Agreement, R-101 Fire Supression System, between the Town and Safety Consultant Inc., for inspection services and/or maintenance of the dry chemical fire control system at the Safety Storage (STOP) Building at the Landfill Site, Cutchogue, New York,- in the amount of $450.00 per year for two inspections, and $325.00 for recharging the system when needed. In consideration of $ /~.~0.00 per year'to be paid by Purchaser to Distributor, Distributor agrees to perform inspection services and/or maintenance of the Ansul R-101 System(s) described below, as recommended by The Ansul Company and required by National Fire Protection Association Standard No. 96. Distributor agrees to conduct no less than two (2) inspections within one (1) calendar year of the date of this Agreement, each inspection to be made at approximately six (6) month intervals. System No. 3 '-System No. 4 - System No. 5 WOO be billed per inspection - $225.00 - Upon completion of each inspection of the above referenced Ansul R-101 Systems, the following discrepancies, if any, in maintenance or installation will be brought to the attention of the Purchaser or his agent, employee, or representative in writing: "1· Damage to the aforementioned Ansul R.101 System including: a. Broken or loose nozzles; b. Bent or insufficiently supported systems piping; c. Restrained or obstructed cable (wire rope) movement· 2. Visible changes, at the Ansul R-101 location, from the original installation survey which will prevent the operation of the aforementioned Ansul R-101 unit to include: a. Relocated cooking appliances; b. Substituted cooking appliances; c· Modifications to the ventilation system; d. Severe grease or contaminant accumulation. Distributor hereby advises Purchaser, and Purchaser hereby acknowledges that damage to the Ansul R-101 System or changes at the Ansul R-101 location from the original installation survey may result in failure of the System to activate under fire conditions. When required, necessary repairs or alterations should be undertaken immediately to ensure the reliability and efficiency of the Ansul R-I01 System. Charges associated with necessary repairs or alterations are not covered by this Agreement· · Re~:harging of an Ansul R-101 system, including lab.or and material, will be billed separately at $ 325.00 per system. The Ansul R-101 System(s) shall be serviced Within twenty-four (24) hours of notification of system actuation. The signatures below signify mutual acceptance of the agreement· It continues in effect for one year and from year to year thereafter at the annual charge in effect at the date of renewal, unless cancelled by written notice by either party, at least 30 days prior to the new agreement year. Annual rates are subject to'change only at the time of renewal.
