RESOLVED that the Town Board of the Town of Southold hereby authorizes Supervisor Jean
W. Cochran to execute a contract with Phone-Review, Inc. to report discrepancies to the
telephone company and obtain credit for thc town.

I'm sure you are very busy this time of the year because of the upcoming Election, but I think it would be wo~h
it reading this. Every town and village on Long Island can surely use a rebate from the telephone company.
Phone-Review has a strong presence throughout Long Island, having done audits for numerous municipalities as
well as large volume accounts. Our major clients on Long Island include Winthrop Hospital, Saint Charles
Hospital, Eastern Suffolk BOCES, Nassau BOCES, and the Pall Corporation.

A few things to remember about our service:
** You supply us with one month's worth of billing and we will do thc entire review from our office. No need to

** No charge if we do not obtain a credit or refund. If we do obtain a credit for you, our fee is simply 50% of the
amount recovered. There are no other charges.

The exempt organization must give you certJfica~on at the time
of the organization's first purchase. A separate document is not
necessary fur each subsequent purchase, provided that the
exempt organization's name, address, and ceraficate number
appear on the sales slip or billing invoice. The certification is
considered part of each order and remains in force unless
revoked.
If a certification with all enffias completed is net received
within 90 days after the delivery of the properly or service,
you will share with the purchaser the burden of proving the
sale was exempt.
You must keep this Exempt Organization Certification for at least
three years after the date of the test exempt sale substantiated
by the certification.

Hospitals that have been granted an exemption from sales~and
use tax pursuant to section* 1116(a)(4) of the Tax Law may claim
exempUon on the purchase of motor fuel by using Form FT-937,
Certificate of Sales Tax and Motor Fuel Tax Exemption for
Qualified Hospitals.
Sub~tandaJ civil and/or criminal penalties will result from the
misuse of this form.
