This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, stroets, and unusual natural or
topographic features.
2. Final Approval froniHealth Dept. of water supply and sewerage-4isposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 19S7) non-conforming uses, or buildings ~md ,pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is

a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Hot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Pemfit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

SEQRA DETERMINATION: The Zoning Board of Appeals has visited the
property under consideration in this application and determines that this
review falls under the Type II category of the State's List of Actions, without
further steps under SEQtLA.
SUFFOLK COUNTY ADMINISTRATIVE CODE: This application was
referred as required under the Suffolk County Administrative Code Sections
A 14-14 to 23, and the Suffolk County Department of Planning issued its
reply dated September 22, 2008 stating that this apphcation is considered a
matter for local determination as there appears to be no significant county-
wide or inter-community impact.
LWt~P DETERMINATION: This application is exempt under Chapter 268-F.
PROPERTY FACTS/DESCRIPTION: The subject property is a 5,000 sq. ft.
non-conforming lot improved with a single family dwelling and accessory
garage. This lot has two front yards and no rear yard, measures 50 feet wide
on Camp Mineola Road, 100 feet wide on Fay Court (to the west), and is 100
feet deep.
BASIS OF APPLICATION: Request for a Variance under Sections 280-122A
and 280-1244, based on the Building Inspector's August 5, 2008 Notice of
Disapproval concerning proposed additions and alterations to an existing
seasonal dwelling for conversion to a year-round residence, which new
construction will increase the lot coverage in excess of the code-limitation
(maximum) of 20 percent.

5. Town Law §267-b(3)(b)(4). No evidence has been submitted to suggest
that a variance in this residential community will have an adverse impact on

6. Town Law §267-b. Grant of the requested relief is the minimum action

RESOLUTION OF THE BOARD: In considering all of the above factors and
applying the balancing test under New York Town Law 267-B, motion was

residence comply with the NYS Euergy Conservation and Construction Code. The
residence is presently being renovated to include a new kitchen and a new bath and also a
new oil fired heating system.
We make the following recommendations:
1. Ail existing windows are to have storm sash for both floors.
2. Provide a storm door for the door to the porch.
3. Install unlaced fiberglass batt insulation over the crawl/cellar, R-19.
4. Insulate all ex/sting exterior walls of the first floor and the gable walls of the
second floor with Fiberglass Blowing Insulation at the rate of a minimum pounds
per square feet of.46 for a R value of 14.
5. Insulate ceiling of the fa'st floor under the unheated attic (from the knee wall to
the eaves) with the same fiberglass blowing insulation at a value of R-14.
6. Two (2) new windows are to be installed which are to be Andexson low E
insulated glass.
7. Insulate the knee walls with 3-1/2" thick fiberglass bart insulation for a R value of
