TOPIC DISCUSSION ACTION
Review of Previous Minutes Minutes from 3/31 meeting were reviewed and accepted.
Tour and Overview: Medical Louise Blackburn and Judy Sherman RN Manager provided a tour and
Model Day Care overview of the new medical model day care at San Simeon. The
program is set to open on 5/3. It will provide a full array of medical and
ADL services to individuals 18 and older, Monday-Friday 9:00am-
4:30pm. Louise will assess each patient prior to admission and will also
provide in home assessment. The program is reimbursed via Medicaid,
long term care ins or private pay. Jay Slotkin, MD is the medical
director.

care. She also suggested she and Louise team up to provide education to
local groups. Karen and Holly will be going to Fishers Island to offer
senior Citizens ID’s. She noted that the Town has approved a PT aide
for the community center on FI.

5/5 Senior Advisory Day5/7 Senior Prom. May
is Older American’s Month as in previous years the center will be
hosting a health fair. This year’s theme is Age Strong, Live Long, the
date for the fair is 5/26/10. Additional programs include Preventing
Falls, CPR and First Aid Safety and Safe Medication Use.
Phyllis Markopoulos
: Working collaboratively with Senior Options
and Solutions to identify potential mental health issues and refer for
assessment.

: Caregiver meeting is going well. Ray Krupski from

ELIH spoke at the last meeting and was very well received. The 2
annual luncheon is coming up.
Mental Health Integration Next Wellness series 5/7, Your Aging IQ, How Much do You Know Karen McLaughlin will be setting up a
Grant About Aging. Updated the group on the status of the telepsych breakfast with Barbara Chandler from the
Senior Options and Solutions programs. Discussed educational programs offered through LIGEC and GEC to explain the Associates Program
becoming and Associate. and encourage participation from local
professionals who work with seniors.
Jay Slotkin, MD:
Agency Reports Nursing Home is full at Peconic Landing resulting in
patients being admitted to San Simeon. He reports that Dr. HSU (one of
our only psychiatrists to refer to) in Riverhead is leaving, unsure of a
replacement.
Daphne Shuttleworth
: Health Ins. 101 will be provided at the
Riverhead library on 5/24 from 6-7pm.
Bruce Petrucci
: Volunteer recruitment efforts continue. Discussed the
potential of using SKYPE for friendly visiting.
Wayne Miller and Barry Goodman
: Wayne reports that they are the
first company in Suffolk County to be approved by Medicaid for
ambulette stretcher transport. He discussed the difference between
ambulance and ambulette transport.
Holly Rhodes Teague
: HEAP will close 5/15. Touro Law is now
providing legal services to individuals over 60 in Suffolk County.

Stephanie Pacchiano
: Provided information on CEU credits through
the Lifeline on Fall Related Brain Injury in Older Adults. Also
discussed the success of the new auto alert product.
Julia Graziano
: Discussed services paid through long term care
insurance. Also gave update on the cancer support group, which meets
the third Tuesday of the month at the Southold Presbyterian Church.

Tracy Zaweski
: Services provided through Home Instead include
transportation. There is a 3-4 hour minimum. Some seniors are using the
service on an intermittent basis to get to MD appointments and to do
errands.
Karen DeMairo
: discussed financial aid available for person’s dx with
a blood cancer. Also discussed upcoming program via the LIGEC on
Getting the Best Cancer Care at Age 55 and Older. Juliet has e-mailed
the flyer to the group members.
Roundtable Discussion Our Health and Wellness Fair will take to place of the next
regularly scheduled meeting on 5/26/10 from 10:30am to 1:00pm.
