We represent the owner of the above captioned property. In an
effort to get her Certificates of Occupancy in order, we enclose
the following:
1. Application for Certificate of Occupancy for two accessory
storage buildings. Building Permit no. 8279Z issued on November
5, 1975 for same is enclosed, along with a copy of the paid
receipt for the application fee. Our check for the application
fee for the C.O. in the amount of $25.00 is also enclosed.
2. Original C.O. no. Z4091 issued on December 21, 1970. You
should find once you examine the application for building permit
and plans submitted in connection with this C.O., the C.O. covers
the detached garage and renovations to the dwelling. Kindly
amend the original C.O. by adding same. (The C.O. for the
original dwelling was issued on July 28, 1959 no. Z355, copy
enclosed).
If you have any questions, please give me a call. Otherwise, I
would appreciate your forwarding my request to my office at your
earliest convenience.

1. Final survey of property with accurate location of all buildings, property lines,
streets, and unu:;ual natural or topographic features,
2. Final Approval from H~blth Dept. of wa~er supply and sewerage-disposal(S-9 form),
3. Approval of electricm~ installation from Board of Fire Underwriters,
Sworn statement from~plumber certifying that the solder used in system contails
less than 2/10 of 1% lead.
5. Com~mercial buildinB, industrial building, multiple residences and similar building~
and installations,,a/ certificate of Code Compliance from architect o~ engineer
responsible for th~ building,
Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:

a. This application must be completely filled in by typewriter o~ in ink and submitted in triplicate to the Building
Inspector, with 3 sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and giving a detailed description of layout ofproperty must be drawn on the diagram which is part of this application.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
