Reft WS CMR PREVENTATIVE MAINTENANCE AGREEMENT For: Fishers Island Pump Station The Preventative Maintenance Agreeme.nt for the above mentioned pump station is based on (2) visits. The togal contract for the (2) visits is $2,174.00 The following items will be check during our visit: 1. Check pump operation. 2. Check wet-well for any foreign materials that could be detrimental to thc pumps. 3. Check float operation and cycle pumps. 4. Check alarm conditions. 5. Check motor ampcrages and voltages as not to exceed nameplate recommendations and record in a maintenance log kept inside the control panel. 6. Clean float switches and pumps of foreign substances, (i.e: grease soap build up or anything that could hinder the proper operation). 7. Pumps to be pulled from the wetwcll for determination of wear, inspect and check for debris. g. Complete test of the alarms, contactors, and level controls. Check voltages in contantor coil, and check control voltage. 9. Confirm proper operation of thc control panel. 10. Formal disclosure to be submitted at the conclusion of the inspection detailing current condition of each system and any findings from the inspection. 11. Maintenance procedures to be performed as per manufacturer requirements on the pumps were applicable. 12. Pump out by truck and/or station pomps and thc disposal of thc grease in the wetwcll, PLEASE NOTE: Pump truck in not included in the quote and is provided by others. 13. Pumps to be hoisted and inspected individually top maintain transfer system operation at all times. Documented inspcctinn to bc include thc following: a. Pump impeller and housing b. Pump wiring c. Float and transducer mechanism d. Rail System e. General condition of the system If any repairs are required on the pumps or associated equipment they shall be performed at the normal rate plus parts and material. Please note: The above Preventative Maintenance Agreements represents a maintenance agreement only and does not in any way guarantee the function or the operation of the pump system. Please sign below if you are in agreement and return the Preventative Maintenance Agreement with the annual payment. Payment should be made by check or money order payable to Weston & Sampson, Inc CMR. This offer is extended for ninety (90) days for the date of this letter.
