STHRC, Juliet Frodella, South Oaks, ELIH, Nancy Gray, Private Practitioner, Jay Slotkin, MD, Daphne Shuttleworth, Comfort Keepers, Holly Rhodes Teague SCOFA, Tracy Zaweski, Home Instead, Colleen Merlo, Mental Health Assoc., Stephanie Pacchiano, United Lifeline, Wayne Miller and Barry Goodman, Choice Medical Transport, Jeri Glander, Peconic Landing, Carol Ryan, Wells Fargo TOPIC DISCUSSION ACTION Review of Previous Minutes Minutes from 10/28 meeting were reviewed and accepted. Karen McLaughlin Southold Town Senior : Medicare Part D enrollment 11/15-12/31. Karen Services Report will assist seniors in need, but encourages seniors who are computer savvy to utilize website. Support can also be obtained through HICAP, see schedule for local libraries. The seniors loved the Ti Chi classes and they will likely offer them on a continued basis. “Blizzard Bags” were provided to local seniors containing a 2-day non-perishable food supply. 12/23 they will have their Christmas dinner, 12/30 will be the New Years formal. Jean DeLorme : Katinka has maintained a steady number of participants. Phyllis Markopoulos : Increase in meals on wheels requests. Medical transport very busy, also using Dominican Sisters for overflow. Lynne Richards : 56 individuals participated in the 11/14 trip to see South Pacific and dinner in Little Italy. Last Saturday 97 people participated in the Holiday lunch Mental Health Integration Feedback on the Geriatric Mental Health Conference at Atlantis Marine Grant World on 11/10. Juliet/ South Oaks are making progress towards the Senior Options and Solutions provision of telepsych pilot project with one local PCP. Will also be meeting with the Federation of Organizations regarding expanding their Senior Companion and respite programs to the East End. 2/8 Dr. Barbara Prestano will be doing a program at ELIH on hearing loss and provide free hearing assessments. She is also working with Dr. Gatewood on a memory screening clinic at the senior center. Health Care Issues and Services for Seniors Committee Meeting Minutes Jay Slotkin, MD.: Agency Reports San Simeon hopes to open the new medical model program in Feb/March 2010. He will be presenting at Southampton hospital grand rounds on 1/7 on the medical home model. The Camden group is working with the East End Alliance on a medical group. Nancy Gray : Discussed the importance of using local business and vendors for potential Holiday gifts. Daphne Shuttleworth : Alphabet Soup is a free insurance counseling seminar being offered at the Riverhead Library. Holly Rhodes-Teague : Over 5000 HEAP applications have been processed so far. MI HEAP is a program for middle income, info available on website. Legal Aide is no longer being funded. Tuoro will be providing this service in the New Year. Senior Services will not incur any additional cuts through the end of March. Next budget year begins in April. Colleen Merlo: 2/10 Next Wellness Series. The program will only be offered at 11:00am for next year on the first Friday of the month at the senior center. The series at Hampton Bays Center went well and we will be working with them to offer quarterly presentations. Colleen will also be speaking at the Cancer Support group in Southold on Seasonal Affective Disorder. Stephanie Pacchiano : Coupons for free installations are available. Also provided the group with educational opportunities for free CEUs through Phillips Lifeline. Choice Medical Transport: Provides the Gift of life- free CPR certification. Jeri Glander : Peconic Landing has openings in assisted living. Tracy Zaweski : Discussed the Stages of Senior Care book written by Home Instead founder. Carol Ryan : Reverse mortgages have been challenging due to lower appraisals. Health Care Issues and Services for Seniors Committee Meeting Minutes Service Barriers Roundtable Discussion 1. : Yearly assessment of service barriers These issues will be explored in the discussed. The group identified the following major barriers: coming year. Transportation and geographic challenges of living on the East ?? End. Lack of Psychiatrist located on the North Fork in private practice ?? Affording Long Term Care ?? Overcoming resistance to care recommendations when working ?? with seniors and their families. Case Review and Collaboration: 2. Deferred Special Guest Speaker: 3. Karen will invite Robin Imandt to discuss the North Fork Women for Woman program at the next meeting.
