STHRC, Juliet Frodella, South Oaks, ELIH, Nancy Gray, Private Practitioner, Jay Slotkin, MD, Julia Graziano, Care Management Associates, Daphne Shuttleworth, Comfort Keepers, Holly Rhodes Teague SCOFA, Stephanie Pacchiano, United Lifeline, Louise Blackburn, San Simeon SNF, Robin Imandt, East End Care Solutions, Tracy Zaweski, Home Instead Senor Care TOPIC DISCUSSION ACTION Review of Previous Minutes Minutes from 7/29 meeting were reviewed and accepted. There was no meeting held in August. Karen McLaughlin Southold Town Senior Town budget has been submitted. Karen and Holly Services Report will be returning to Fishers Island on 10/15 to meet with the senior lunch group and to continue to assess needs and develop strategies for Island residents to have access to needed services. Medicare drug plans are going up 20%. Karen encourages seniors who are computer savvy to utilize website. Support can also be obtained through HIICAP, see schedule for local libraries. 220 seasonal flu shots were given yesterday at the Peconic Center by Suffolk County Department of Health (SCDOH). H1N1 vaccinations will be given to priority groups screened by SCDOH. Must call for screening, appt. Dates and locations will be forthcoming. Jacqueline Martinez : Qi Gong is being offered for $5.00 a class. There is a nutrition class provided by Cornell every Wed. First nd Thursday of the month hearing screenings, 2 Thursday each month George Roche, legal clinic Jean DeLorme : Katinka House now has 13 members. Margaret Wisz recently spoke at caregiver support group on Medicaid eligibility. 10/20 at 1:30 Dr Pflaster will speak. Phyllis Markopoulos : 118 people on meals on wheels. Many calls on Medicare Part D coverage as well as upcoming HEAP season benefits. New applicants, 11/2. Previous applicants have already received benefit application for this season. Health Care Issues and Services for Seniors Committee Meeting Minutes Lynne Richards : 11/14 trip to see South Pacific and dinner in Little Italy Mental Health Integration Next Wellness Series on 10/2 Cocktails and Nightcaps: When to Say Grant When. 10/28 Gait and Balance Assessment by Dr. Caroline Gatewood, Senior Options and Solutions ELIH Gladys Brooks Medical Village, call 477-5121 for an appointment. The Arthritis Foundation did a presentation at TOS on 9/22. They will be repeating this program at ELIH on 10/26 at 11:00am. Following a meeting with Holly, Karen, and Southampton Town Senior Services, the mental health wellness education series will be offered at the Hampton Bays site beginning 10/8. We will be providing a 6-week series there with the Mental Health Association. Discussed geriatric mental health needs and the continued lack of psychiatry on the north fork. Juliet/ South Oaks are discussing provision of telepsych pilot project with one local PCP. Jay Slotkin, MD Agency Reports : LIGEC will be providing a program on IV Therapy at home. His office is transitioning to an electronic medical record. New primary care MD, in Cutchogue, Dr. Bellamy Brooks and Dr. Hugglemeyer. Being credentialed at ELIH. Blue Cross Senior plan settled, still in negotiations for others, no other hospital provider within 30 miles, OK, check policy. Nancy Gray : Practice going well Daphne Shuttleworth : Will be having a fundraiser benefiting LI Alzheimer’s Assoc. at Martha Clara Vineyard on 10/11 from noon to 6:00pm (see flyer) Julia Graziano : Care Management Assoc. provides private geriatric care management for seniors and their families. Discussed need for this service. Cancer Support group now has @ 7 participants. Attended Fighting Chance Day of Hope. Holly Rhodes-Teague : Public hearings 10/28 at 10:00am in Flanders, 10/27 in Brentwood. Information on health insurance will be available. Health Care Issues and Services for Seniors Committee Meeting Minutes Discussed status of HEAP. Stephanie Pacchiano : United Lifeline now has a cordless phone available with enhanced range. Robin Imandt : East End Care Solutions, private care management services. Louise Blackburn : Medical model is now slated to open 2/10. It will cost $150.00 per day plus $44.00 for transportation for non-Medicaid clients. The program will run from 9-3, people will be asked to pay a month in advance, while there will be 30 slots, people will not be required to attend daily which will allow for additional enrollees. We will plan to have a future meeting there; all questions can be e-mailed to Louise. Peter McGarry has been hired as a new admission coordinator. Tracy Zaweski : Home Instead Senior Care is non-medical care provider. New office on Hampton Bays covering both the north and south forks. $21/hr, minimum 3 hours, All caregivers are employed, insured, and bonded. North Fork Women for Women will be asked to speak at a 4. future meeting. Our next scheduled meeting will be 10/28/09.
