This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
For new building or new use:
1. Final survey of property with accurate location of atl buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957} non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

a. This application MUST be completely filled in by typewTiter or in ink and subm/tted to the Building Inspector with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Pe~it.
d. Upon approval of this application, the Building Inspector will issue a Building Pe~Tnit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work. · '
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months fi'om such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may author/ze, in writing, the extension of the permit for an
addition six months. Therea~ffer, a new permit shall be required.

ACCESSORY USE AREAS
Any other accessory use area shall not be required to be separated
by a fire barrier, provided the accessory use occupies an area not
more than 10 percent of the area of the story in which it is located
and does not exceed the tabular values in Table 503 for the
allowable height or area for such use.
302.3.1 NONSEPARATED USES
Each portion of the building shall be individually classified as to
use. The required type of construction for the building shall be
determined by applying the height and area limitation for each of
the applicable occupancies to the entire building. The mbst
restrictive of construction, so determined, shall apply to the entire
building. All other code requirements shall apply to each portion of
the building based on the use of that space except that the most
restrictive application provisions of Section 403 and Chapter 9 (see
below) shall apply to these nonseparated uses. Fire Separations
are not required between uses except as required by other
provisions.

the building elements listed in Table 601 are of noncombustible
materials.

EGRESS THROUGH INTERVENING SPACES
Egress from a room or space shall not pass through adjoining or
intervening rooms or areas, except where such adjoining rooms or
areas are accessory to the area served: are not a high-hazard
occupancy and provide a discernible path of egress travel to an
exit. Egress shall not pass through kitchens, storage rooms, closets
or spaces used for similar purposes. An exit access shall not pass
through a room that can be locked to prevent egress. Means of
egress from dwelling units or sleeping areas shall not lead through
other sleeping areas, toilet rooms or bathrooms.
EXIT OR EXIT ACCESS DOORWAYS REQUIRED.
Two exits or exit access doorways from any space shall be
provided where one of the following conditions exists:
1. The occupant load of the space exceeds the values in
Table 1015.1.
2. The common path of egress travel exceeds the limitations
of Section 1014.3.
3. Where required by Sections 1015.3, 1015.4 and 1015.5.
