'This applicatlon must be filed in typewriter OR ink and submitted to the Building Inspector with the following; for new buildings or new use: I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of rater supply and sewerage-disposal(S-9 form). 3. Approval of electrical installation from Board of Fire gnder~rlters. 4. Sworn statement from plumber certifying that solder used in system contains less than 2/10 of ]Z lead. 5. Commercial buildings, industrial buildings, multiple residences and similar buildings and installations, a certificate of code compliance from the Architect or Engineer responsible for the building. 6. Submit Planning Board Approval of completed site plan requirements. B. For existing buildings (Prior to April 9, 1957] non-conforming uses, or buildings and "pre-existing" land uses: I. Accurate survey of property shoving all property lines, streets, buildings and unusual natural or topographic features. 2. A properly completed application, a consent to inspect signed by the applicant and a certified abstract of title issued by a title company ~hich shall show single and separate ownership of the entire lot prior to April 9, ]957. If a Certificate of Occupancy is deuied,'the Building Inspector shall state the reasons therefor in ~riting to the applicant. 3. Date of any housing code or safety inspection of buildings or premises, or other pertinent information required to prepare a certificate. C. For Vacant Land Certificate of Occupancy: ].
