2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval o f electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:

a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of a Building Permit pursuant to the
Building Zone Ordinance of the Town of Southold, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The
applicant agrees to comply with all applicable laws, ordinances, building code, housing code, and regulations, and to admit
authorized inspectors on premises and in building for necessary inspections.

9. THE CONTRACTOR SHALL VERIFY AND BE RESPONSIBLE FOR

ANY DISCREPANCIES IN THE EXISTING CONDITIONS FROM
THOSE SHOWN ON THE CONTRACT DOCUMENTS TO THE
/ENGINEER OF RECORD.
10 PRIOR TO EXCAVATION, THE CONTRACTOR SHALL CONTACT
THE UTILITY COMPANIES TO LOCATE/MARK-OUT ALL
UNDERGROUND FACILITIES AND OBTAIN CASE #. THE
CONTRACTOR SHALL ALSO LOCATE ANY OVERHEAD UTILITIES
AND DETERMINE THE REQUIRED CODE CLEARANCE PRIOR TO
OPERATING ANY EQUIPMENT.
11. ALL EXCAVATIONS SHALL BE IN ACCORDANCE WITH SAFETY
REQUIREMENTS OF THE OCCUPATIONAL SAFETY & HEALTH
ADMINISTRATION (OSHA) REGULATIONS.
12. ANY MACHINE EXCAVATION SHALL BE LIMITED TO WITHIN 3"
INCHES OF THE PROPOSED FOOTING SUB-GRADE WITH THE
REMAINING REMOVAL DONE BY HAND EXCAVATION TO
MINIMIZE SOIL DISTURBANCE.
13. ALL FOUNDATIONS AND FOOTINGS ARE SIZED FOR BEARING
ON VIRGIN SOIL AT MINIMUM BEARING CAPACITY OF 2 TONS
PER SQ. FT. AND MINIMUM OF 4 FT, COVER. SOIL AT SUB-
GRADE ELEVATION MUST BE INSPECTED AND VERIFIED BY
CONTRACTOR THAT FOUNDATION SOILS HAVE THE HEREIN
SPECIFIED MINIMUM BEARING CAPACITY.
14. ALL FOUNDATION FOOTINGS AND SLABS MUST BEAR ON
UNDISTURBED SOIL. ALL SOIL DISTURBED DURING
EXCAVATION MUST BE COMPACTED TO 98% DENSITY IN
ACCORDANCE WITH PROJECT SPECIFICATIONS. IF DISTURBED
SOILS ARE FOUNDATION SOILS FOR ANY PORTION OF THE
BUILDING, CONTRACTOR MUST PROVIDE SOIL TESTING REPORT
VERIFYING SOIL COMPACTING TO SPECIFICATIONS.
15. CONTROLED LOW STRENGTH MATERIAL (CLSM) OR FLOWABLE
FILL MAY BE USED IN THE AREAS WHERE COMPACTION OF SOIL
CAN NOT BE ACCOMPLISHED. CONTACT OF ANY ALUMINUM AND
CAST IRON MATERIAL WITH THE CLSM IS PROHIBITED.
16. ALL CONCRETE SHALL HAVE A 4,000 PSI MIN. COMPRESSIVE
STRENGTH AT 28 DAYS AND BE AIR ENTRAINED IN ACCORDANCE
WITH SECTION R402.2 OF NEW YORK STATE RESIDENTIAL CODE.
CONTRACTOR TO PROVIDE VERIFICATION, AS REQUIRED, TO
DETERMINE THE CONCRETE STRENGTH.
17. ALL STEEL REINFORCEMENT SHALL BE GRADE 60 (Fy=60 KSI).
18. POURED CONCRETE SLABS TO BE PLACED OVER WELL
COMPACTED SANDY MATERIAL.
19, FOR ALL 6" POURED CONCRETE SLABS, PROVIDE 6'x6' 10X10 WWF
OVER 6 MIL POLYETHYLENE VAPOR BARRIER OVER 12" OF WELL
COMPACTED SANDY FILL.
20. ALL STRUCTURAL STEEL SHALL BE A.S.T.M A36 DESIGNED,
FABRICATED AND CONSTRUCTED IN ACCORDANCE WITH THE
A.I.S.C. STANDARDS. ALL STEEL MUST BE CORROSION PROTECTED
BY HOT-DIP GALVANIZATION.
21. ALL CONSTRUCTION SHALL BE IN ACCORDANCE WITH THE NEW
YORK STATE CODES VERSION OF THE IBC 2000 CODES.
22. IT IS THE RESPONSIBILITY OF THE OWNER/CONTRACTOR TO
EXPOSE AND INSPECT THE CONDITION OF THE EXISTING
FOUNDATION, DAMAGED/DETERIORATED AREAS SHALL BE
REPAIRED/REPLACED IN ACCORDANCE WITH THE APPLICABLE
STATE AND LOCALCODES AND AS APPROVED BY A LICENSED
ENGINEER.
23. THE CONTRACTOR SHALL STAGE HIS OPERATION AND TAKE
NECESSARY MEASURES TO PREVENT STORM WATER RUNOFF OR
GROUND WATER FROM ENTERING EXCAVATION AREA, SITE TO BE
PROTECTED DAILY. GRADE SITE PERIMETER AWAY FROM HOUSE /
EXCAVATION AREAS AND PREVENT STORM WATER RUNOFF FROM
DAMAGING LANDSCAPED AREAS OR ADJACENT PROPERTIES.
DEWATERING MAY BE REQUIRED DUE TO THE CLOSE PROXIMITY
TO THE CANAL AND TIDAL WATERTABLE. CONTRACTOR TO APPLY
AND OBTAIN DEWATERING PERMIT IF APPLICABLE.
24. NO CHANGES SHALL BE PERMITTED WITHOUT PRIOR WRrl-rEN
NOTIFICATION OF AND WRTTEN APPROVAL OF THE ENGINEER OF
RECORD.
25. ANY OMISSIONS IN THE PLANS IDENTIFIED BY OWNER OR
BUILDING FOUNDATION, A MINIMUM OF 8 FOOT HRIZONTAL
CLEARANCE OR AS SHOWN ON THE PLANS SHALL BE
MAINTAINED DURING THE EXCAVATION AND CONSTRUCTION
OF THE LOADING DOCK FOUNDATION.
29. ALL DIMENSIONS AND ELEVATIONS WERE TAKEN FROM THE
OWNER PROVIDED SURVEY OR HIS FIELD MEASUREMENTS. A
COMPLETE SURVEY SHOWING GROUND ELEVATIONS IS
REQUIRED FOR PROPER CONSTRUCTION.
