a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of bu/ldings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months afier the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Additions and Alterations to the Peretz Residence, 5796 Rocky Point Road, East Marion
FRAMING
The framing of the above referenced alterations was performed as per revised plans dated June 10,
2008.
The following changes have been implemented:
New windows were upgraded to all mahagony Green Mountain Windows as opposed to
Andersens, all 1'" floor double-hungs needed to be reduced in size;
Bedroom #5 behind the kitchen was eliminated in favor of a larger BR #4 and separate pantry,
the bathroom #3 was relocated;
Part of the deck facing west by the mudroom was covered and will be used as a screened porch;
Part of the main roof above the loft area was raised to accommodate vaulted dormers;
All insulation in exterior walls and roofs was upgraded to sprayed foam insulation (Icynene) which
raises the R-value to ~6.2 per inch and improves protection against air infiltration.
STRAPPING
The strapping details were executed as per architectural plans and specifications.
ROUGH-IN PLUMBING
All rough-in plumbing was completed before the insulation was installed.
Pressure tests showed satisfactory results, no leakage was detected.
FIRE AND PERIMETER CAULKING
All necessary caulking was done as per code.
