A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or

2. Final Approval from Health Dept. of water supply and sewerage-dispos, al (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.

of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

features.
2. A properly completed application and a consent to inspect signed by the applicant. Ifa Certificate of Occupancy

AUTHORIZATION
Pursuant to the provisions of Chapter 275 and/or Chapter 111 of the To~w~ Code of the Town of
Southold and in accordance with the Resolution of the Board of Trustees adopted at the meeting held on
June 20, 2007, and in consideration of application fee in the sum of $250.00 paid b3
subject to the Terms and Conditions as stated in the Resolution, the Southold Town Board of Trustees authorizes
and permits the following:
Wetland Permit to demolish and remove an existing 2 ~ story dwelling and
appurtenances, including a tennis court, and construct in new, landward location a two-
story, one-family dwelling with attached porch, patio, deck, pergola, and other as
appurtenances, including an in-ground swimming pool; install a drainage system of
drywells with incorporated concrete retaining wall; erect a fence; install a fish pond; and
truck in approx. 26 cy. clean sand fill to be incorporated into cut and fill operations
required to achieve proposed grades, and with the condition a 15' non-turf buffer is
installed landward of the crest of the bluff, a row of staked hay bales with a silt retention
fence is in place prior to the start of construction, and all as depicted on the survey
prepared by Nathan Taft Corwin III last dated July 11, 2007.
IN WITNESS WHEREOF, the said Board of Trustees hereby causes its Corporate Seal to be affixed,

determined that the two-story single-family dwelling with gutters, leaders to drywells was
constructed in compliance with Wetland Permit #6654, as issued on June 20, 2007. All
other construction through Wetland Permit #6654 has not been completed at this time.

The relhrenced property landward of the crest of a blurt; labeled "Top of Slope" in excess of 10
feet in elevation as shown on the survey prepared by Joseph A. lngegno last revised 2/13/07 is
beyond Tidal Wetlands Act (A~licle 25) jurisdiction. Therefore, in accordance with the cut'rent
Tidal Wetlands Land Use Regulations (6NYCRR Put 661) no permit is required.
Be advised, no construction, sedimentation, or disturbance of any kind may take place
seaward of thc tidal wetlands jurisdictional boundary, as indicated above, without a pemfit
It is your responsibility to ensute that all precautions are taken to prevent any sedimentation or
other alteration or disturbance to the ground surface or vegetation within Article 25 jurisdiction
which may result fi`om your project. Such precautions may include maimaJning adequate work

d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
