This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to respect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Metal connector plates shall be applied on both faces
of truss at each joint. Center the plates, unless
indicated otherwise. No loose knots or wane in plate
contact area. Splice only where shown. Overall spans
assume 4" bearing at each end, unless indicated
otherwise. Cutting and fabrication shall be performed
using equipment which produces snug-fitting joints and
plates. Unless otherwise noted, moisture content of
lumber shall not exceed 19% at time of fabrication and
the attached truss designs are not applicable for use
with fire retardant lumber and some preservative
treatments. Nails specified on Truss Design Drawings
refer to common wire naris, except as noted. The
attached design drawings were prepared in accordance
with" National Design Specifications for Wood
Construction" (AF & PA ), "National Design Standard
for Metal Plate Connected Wood Truss Construction"
(ANSI/TP1 1), and HUD Design Criteria for Trussed
Rafters.
Robbins Eng. Co. bears no responsibility for the
erection of trusses, field bracing or permanent
truss bracing. Refer to "Building Component Safety
Information" (BCSI 1) as published by Truss Plate
Institute, 218 North Lee Street, Suite 312, Alexandria,
Virginia 22314. Persons erecting trusses are cautioned
to seek professional advice concerning proper erection
bracing to prevent toppling and" dominoing ". Care
should be taken to prevent damage during fabrication,
storage, shipping and erection. Top and bottom chords
shall be adequately braced in the absence of sheathing
or rigid ceiling, respectively. It is the responsibility of
others to ascertain that design loads utilized on these
drawings meet or exceed the actual dead loads imposed

accordance with the site plan approved on July 12, 2007.

a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the pertrdt for an
addition six months. Thereafter, a new permit shall be required.
