This application must be filled in by typewriter or ink and submitted to the~Building Department with the followins:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features. °
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 fom~).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1 % lead.
S. Conm~ercia] building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and wmsual natural or topographic
featw es.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

David Horton,being duly sworn, deposes and states as
follows:
That I have known Richard Keogh since childhood and
also knew his father and am familiar with the Keogh property
located at 75 Edwards Lane, Orient, New York. I am aware
that the wooden structure with utilities located on the
north side of the Keogh property as shown by the survey of
Stanley J. Isaksen, Jr. dated 7/12/07 contained a
residential unit on the east end thereof. Said area was
commonly referred to by the Keogh's as the "studio" and was
used for residential purposes since about 1953 and has been
used for residential purposes on an uninterrupted basis from
the early 1950's to date. I am aware that said premises
were heated until approximately 9 years ago with Kerosene
heat and about 4 years ago Richard Keogh converted to
propane heat.
