This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Colmnercial building, industrial buildiug, multiple residences and similar buildings and installations, a certificate
of Code Compliauce from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirmnents.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or Imihlings and "pre-existing" land rises:
1. Accurate survey of property showing all property lines, streets, building and nnusual natural or topographic
features.
2. A properly compleled application and consent to mspecl signed by the applicant. If a Ccrlilicate of Occupancy is
denied, the Buikling Inspector shall state the reasons therefor in writing to ihe applicant.

Dear Sir:
Please be advised, a Building Permit is required for the conversion of the
dwelling to a year round residence. According to the Code of the Town of
Southold, a Building Permit is required prior to any construction taking place. As
per our discussion on July 14, 2008, please submit a Building Permit application,
specs for heating system, energy code calculations and a check for $400.00.

Dear Mr. Verity,
We have reviewed the installation of a Slant/Fin cast iron boiler at the above referenced
residence.
The boiler is a Liberty Model L40 hot water boiler with a Beckett Flame Retention Head
Oil Fired Burner (Model AFG-SF-11-02).
The heating for this building is distributed through three (3) zones, with individual
circulator pumps and controlling thermostats.
We find that the installation has been installed in a good workmanship manner and
conforms to all Code requirements.
Photographs of the installation and copies of the equipments brochures have been
included herewith for your reference.
If I can be of further assistance, please do not hesitate to call.

form to the mquirementa of the authority having jurisdiction
or, in the absence of such requirements, to the Installation
of Oil Burning Equipment, ANSI/NFPA 31, latest edition,
and to the National Electrical Code ANSI/NFPA 70, latest
edition. The installation must also conform to the additional
requirements in this Slant/Fin Instruction Manual. Where

Product Overview
Dow Chemical's "Styrofoam" brand name encompasses a wide range of polystyrene-based insulation
products, Included in our testing were two versions, the common white "expanded" product and the
'extruded" Styrofoam Square Edge (see photo).
Styrofoam (expanded)- This is the product most people think of when they hear "Styrofoam", It is
white in color and comprised of thousands of individual "beads" which ara pressed together. The
most common use is as a cushioning marshal in shipping and packaging. Although it is not intended
for use as an insulation, it frequently is. Most people have seen cheap coolers and ice chests made
from it. As an insulation, expanded Styrofoam has many qualities which count against it, At R 3.84
per inch, the product is simply not a very good insulator. Secondly, it is very hygroscopicity (absorbs
and holds moisture). This tendency to get, and stay wet, makes it something to definitely avoid
when looking at ice box insulation.
Styrofoam Square Edge (extruded) - This product is often called "blueboard" due to its light blue
color. In spite of its ralatively modest "R" value (4.92 per inch), Styrofoam SM is our overwhelming
recommendation for foam ice box insulation in marine applications. Unlike its expanded cousin
(above)and virtually all other foam insulation, Styrofoam "Square Edge" is completely impervious to
moisture. This is a huge plus. While many other foams start out as better insulators, they inevitably
suffer significant decline as they absorb moisture from the surrounding air. This is not the case with
Styrofoam SP. Another benefit of its hydrophobic nature is that no "air gap" is requ red when
installing it. This means that more insulation can be packed into a smaller space. If is available in
many thicknesses and two sheet sizes.
Test Conclusion
As the f'n~h largest chemical company in the world, Dow does not need to oversell their products or
exaggerate their qualities. Unfortunately, misapplication of expanded Styrofoam has led many people
to think of the brand as offering poor insulation value and high moisture absorption. As previously
stated, expanded polystyrene is not a suitable insulation for moist environment. However, the
extruded version, Styrofoam Square Edge, is another story entirely and we recommend it highly
whenever a standard insulation foam is required.
