MIRIAM BLOCK and WILLIAM BLOCK, residing at 5227 Melbourne
Road, Raleigh, North Carolina 27606 , Co-Guardians of Carla Jean
Block, make the following statement:
1 . We are the Co-Guardians of Carla Jean Block, having been
appointed by Order of Justice A. Gail Prudenti, dated January 10 ,
1997 .
2 . We make this statement identifying the real property of
the Alleged Incapacitated Person pursuant to the Order of Justice
Prudenti .
3 . The Alleged Incapacitated Person is the owner in fee of
the premises located at 2875 Hyatt Road, Southold, New York. Such
property is more particularly described as follows:
ALL that certain plot, piece or parcel of
land, situate, lying and being at Horton ' s
Point in the Town of Southold, County of
JQQ 0 Suffolk and State of New York, being more
particularly bounded and described as follows :
