This permit application should be sensitive to the unique visual and historic setting of the Long
Island North Shore Heritage Area, which runs from the Queens/Nassau County Line to Orient
Point and from Route 25 or I495, whichever is farther south, north to Long Island Sound.

include the County tax map number.

Dear Mr. Smith,
As consulting architect for Mr. David Cifarelli (Crystal Clear Window Cleaning) and
pursuant to our phone conversation, we are herewith submitting four site plans for your
preliminary review. Kindly look over the attached site plans and return same to this
office with your comments as soon as possible. Additionally, kindly forward a formal
application to us with the State's requirements for bonding as well.
The owner Mr. Cifarelli, would like to move on this immediately. We have made
application to the Town of Southold for site plan approval. This includes re-configuring
the addition out to the rear of the building. These permits are imminent.
As I stated in our phone conversation, This is a pre-existing building/site with
approvals for as both residemial and commercial use. It is the intent of Mr. Cifarelli to
begin construction of the addition and site work as soon as praetieal.

We have just closed on a piece of property at 11160 Main Road in Mattituck tax map number 1000-122-03-09
which we plan ~o use for our offlec (window cleaning) and a one t'~mily dwelling, as it has bc~n being used for.
Prior to contract I had been in contact with Mr. Forester who advised me on a few issues. It has come to my
altcntion this morning that I might have been advised incorrectly.
We am r~quosting approval to remove some trees in the back of the property, (please see enclosed survey) re
allow for adequate parking for our company vans. ThL~ parking area will [v~ a stone blend surface with adequat~
dry wells. (We have already contacted A~co Drainage to install the needed wells, ifyou need any intbrmafion
from tl~cm, please let me know.)
We are planning on improving the look of thc building m~d the property. As we fccl that in our business
(window cleaning) that appearance is everything. We, have contacted a few architects to help with tile
appearauee of'the building as well its a Imldscape archix,,'ct for thc property.
