McLean Associates, PCRESOLUTION 2008-895 ADOPTED DOC ID: 4271 THIS IS TO CERTIFY THAT THE FOLLOWING RESOLUTION NO. 2008-895 WAS ADOPTED AT THE REGULAR MEETING OF THE SOUTHOLD TOWN BOARD ON SEPTEMBER 23, 2008: RESOLVED that the Town Board of the Town of Southold hereby authorizes and directs Supervisor Scott A. Russell to retain L.K. McLean Associates~ P.C. to perform environmental due diligence services in connection with the proposed purchase of the premises known as SCTM #1000-113-12-14 (adiacent to Strawberry Fields in Mattituck), pursuant to their Proposal dated August 29, 2008, at a cost not to exceed $2,720.00, subject to the approval of the Town Attorney. McLean Associates, P.C. (LKMA) is pleased to submit a cost proposal for professional services relative to the scope of work discussed with your office on July 30, 2008. Pursuant to the above referenced conversation we understand our scope of services is to include the following; Perform a Phase I Environmental Review of the Caraftis property situated in Mattituck (north of County Road 48). Currently the 1.15 acre parcel is vacant and used as an in-formal access roadway from County Road 48 to the adjoining County parcel (Strawberry Fields). This assessment will consist of a review of existing information regarding past and current land use to identify possible sources of contamination within and around the project site as well as an inspection of the premises. Task #1 - Perform Phase I Environmental Review Review and Research the following: NYSDEC Records, Federal CERCL1S list, Historical aerial photographs, Sanborn Insurance Company Maps, County agency data bases - 2 hours by Environmentalist. Separate cost of $700.00 for record search and environmental lien evaluation by sub-consultant. Perform Site Visit to the parcel and neighboring area to look for observable physical evidence of contamination (e. g., stained soil, seepage, and stressed or dead vegetation). It is assumed that authorization to access the site will be arranged by the Town of Southold - 4 hours by Environmentalist. Prepare a screen report and plan to include the results of the database searches, analysis of the information collected, and observations of the field visit - 6 hours by Environmentalist, 3 hours by Draftsman and 2 hours by Project Manager. · Founded in 1950 Our fee for the above referenced professional services is based upon our existing Annual Professional Services Contracts with other local municipalities and past projects performed for the Town of Southold. Task # 1 - 2 hours by Project Manager @ $125/hr 12 hours by Senior Environmentalist @ $120/hr. 3 hours by Draftsman @ $110/hr. Separate Cost for Record Search = $25O.00 = $1,440.00 = $330.00 = $700.OO Subtotal for Task #1: = $2,720.00 Total Estimated Cost of Professional Services = $2~720.00 An appropriation of $2,720.00 would be required for the estimated engineering services associated with the execution of Task #1. The appropriation would be a cost not to exceed and shall include all out of pocket expenses associated with travel, materials and reproduction costs. Any tasks not identified in this proposal or listed as "optional" shall be considered unauthorized additional work. No additional work shall be performed by our office without proper authorization from the Town of Southold. Should this proposal be acceptable, please sign both copies of same and return one copy to this office. This shall constitute our Notice to Proceed.
