APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, propert)' lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
.3. Approval of electrical installation from Board of Fire Underwriters.
4. Swam statement from plumber certifying that the solder used in s)'stem contains less than 2/10 of 1% lead.
S. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing bnildings (prior to April 9, 1957) non-conforming nses, or buildings and "pre-existing" land uses:
I. Accurate surve)' of propel1)' showing all propert)' lines, streets, building and unusual natural or topographic
features.

Upon request of the Southold Town Building Department,
I made inspection of this one story, three bedroom dwelling
and found the following violations of Local Law #1, Housing
Code of the Town of Southold. I picked up the key from
F. W. Kaelin Real Estate, Main Road, Cutchogue, New York.
The key was for the garage door which is in the basement,
inspection began here at approximately 11:40 a.m.
Basement: Outlet located near fuse box, no plate
and insulation of wire is damaged - Section 528a.
Stairway: From basement to upstairs living quarters -
no light switch for safe ascent - Section 2l5c.
Kitchen: Outside entrance from deck - no light switch
to control light in room - Section 529b.
Living Room: Two outside entrances, no light switch
to control light in room on entry - Section 529a.
East Entrance: Broken pane of glass in dead-lite -
Section 302b.
House heated suitable for year round occupancy - Inspection
completed at approximately 12:15 p.m.

installations, a certificate of Code compliance from the Architect ar Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

FRANCES SEEWALD, being duly sworn deposes and says that
she is the owner of premises known as "Lot No. 53, Map of Section
B, Nassau Point Club Properties, Inc.", at Nassau Point, Southold
Town, New York. She acquired the property from Bertha Brown by a
deed dated November 7, 1940, recorded in Suffolk County Clerk's
office on November 9, 1940 in Liber 2133 of deeds at page 224.
At that time the bungalow on the property had already been con-
structed. No change in t he exterior lines of the dwelling on the
property has been made since the property was acquired in 1940
excepting that prior to 1957 your deponent and her husband changed
the stairs from in front of the bungalow going toward Little Peconic
Bay to the side of the bungalow, where they now are. No changes in
the exterior lines of the bungalow on the premises have been made
since March 1, 1957.
This affidavit is made for the purpose of supporting a
Certificate of Pre-existing Use covering the premises for use in
connection with the current sale of t he same by your deponent.
