ADOPTED AT THE REGULAR MEETING OF THE SOUTHOLD TOWN BOARD ON
JULY 29, 2008:
RESOLVED that the Town Board of the Town of Southold hereby authorizes and directs
Supervisor Scott A. Russell to execute the Memorandum of Understandin¢ amone The
Lone Island Home d/b/a South Oaks Hospital, The Mental Health Association of Suffolk
Countv, Inc., the Center for Essential Management Services, Eastern Lone Island Hospital,
and the Town of Southold in connection with an Office of Mental Health Grant to provide
services in an effort to integrate physical and mental health systems available to the community,
subject to the approval of the Town Attorney.

Oaks Hospital (hereinafter the "SOH"), located at 400 Sunrise Highway, Amityville, NY 11710 and
The Mental Health Association of Suffolk County, Inc., Lindenhurst, NY (hereinafter "MHASC");
Center for Essential Management Services, Amityville, NY (herinafter "GEMS"); Eastern Long
Island Hospital, Greenport, NY (herinafter "ELIH"); and Town of Southold Senior Services,
Mattituck, NY (herinafter "TOS").
Whereas South Oaks is an article 31 hospital, licensed by the Office of Mental Health and the
Office of Alcoholism and Substance Abuse Services of New York State, which provides treatment
for people with psychiatric and chemical dependency illnesses in inpatient and outpatient settings.
Whereas South Oaks is a psychiatric hospital specializing in providing care for the geriatric
population.
Whereas South Oaks has been awarded a grant by the New York State Office of Mental Health to
improve healthcare for the elderly living in the underserved area of the North Fork of Long Island.
Whereas all parties desire to create a collaborative relationship to provide more resources to
individuals and families in the region.
Whereas all parties desire to integrate physical and mental health services, by creating cooperative
links between mental health systems and physical health caze providers/entities.
Now, therefore, it is mutually agreed that the following will take place:
LENGTH OF PARTNERSHIP
The partnership will run for approximately five years, from 2007-2012, provided funding is
continuous from the New York State Office of Mental Health. Partners include:
South Oaks Hospital
Eastern Long Island Hospital
Town of Southold
The Mental Health Association in Suffolk County, Inc.
Center for Essential Management Services
PROJECT DESCRIPTION & PURPOSE
The purpose of this project is to integrate physical and mental health service providers, by creating
cooperative links between mental health systems and physical health care providers/entities. Staff

within each system will have the opportunity to establish linkages to help individuals and families
prevent, detect, and address mental health problems. The system by which stronger linkages will
be created will come out of the collaborative relationships enhanced and further developed under
this grant. The focus of this program is to effectively integrate physical and mental health
practitioners by having Township, hospital- based, and mobile mental health services available.
The project will provide for mental heath screening, assessment, educational materials, referral to
mental health service providers, and coordinate the integration of service systems. All partners
have made resources readily available, including but not limited to, time, personnel, equipment,
space and expertise.
Overall, creating a collaborative relationship between all partners will provide more resources to
individuals and families in the region, enhance access to quality mental health services, and
improve the overall physical and mental well-being of those suffering from mental health issues.
GOALS AND OBJECTIVES FOR ALL PARTNERS
I . Improve access to mental health services through free transportation/home visits
2. Promote independent living and a sense of self sufficiency
3. Encourage community member interaction and friendships
4. Increase the integration of physical and mental health systems
5. Decrease hospitalization
Specific objectives and activities associated with those stated goals include but aze not limited to:
1. coordinating and sharing services between physical health and mental health providers
2. sharing resources to promote recruitment of staff and volunteers
3. prioritizing service area development by identifying the available resources
4. educating the residents and families about services available in the community
5. educating health care and community providers regarding the Health Issues Committee and
its resources
RESPONSIBILITIES AND EXECTATIONS OF THIS AGREEMENT
1. South Oaks Hospital will act as the lead agency in this funded initiative. South
Oaks will direct collaborative efforts among all partners.
2. All partners, by signing this MOU indicate their commitment to participate with the
lead agency in their implementation and ongoing delivery of the project activities
and services. All such activities, objectives and goals described in the grant for
each partner will be carried out in accordance with the originally submitted
application, unless modified in writing and agreed upon by all parties. Individual
agency responsibilities in carrying out the project include:
^ Financial responsibility for the services that it will provide as part of the
program. Attachment A indicates financial data pertinent to each partner,
and individual partner details will be attached accordingly.

^ Accounting and auditing responsibility pertaining to written records of all
activities and staff time billed under the grant, and making these records
available to the lead agency upon request.
^ Full responsibility and liability for the services provided, including quality,
accountability, and coordination of services;
^ Full responsibility for maintaining the confidentiality of all individuals
served under the grant. Individual participant demographic and health
information will be collected by South Oaks from individuals and partner
organizations, and will be maintained according to HIPAA regulations.

personnel. An additional 1,100 includes increased telephone usage, office supplies,
