Please make sure that your inquiry includes all required documentation. Inadequate submissions or discrepancies cannot be interpreted in your favor. All inouiries must include: 1. A cover letter (or application form) stating the reason a determination is needed; nature of project; address of property in question; name, address and telephone ntwber of landowner; and the same information for any authorized contact person (authorisation letter required). 2. Recent clear photographs (dated) showing relevant conditions at the site. 3. A location map showing the property in relation to local roads and landmarks. 4. Documentation as explained below (I,II, or III). If the projecUproperty is: I. Landward of a substantial man-made structure that is greater than 100 fe°t lono was constructed prior to Austtst 20, 1977, and has remained functional: Provide two copies of a stuvey of the site dated before Auatrst 20. 1977 which the clearly shows the structure (i.e. paved road, bulkhead, seawall, retaining wall, etc.) and confirms its length. Two copies of a plan showing all proposed construction must accompany the survey. Photographs must show entire structtue.
