Tliis application must be filled in by typewriter or ink and submitted to the Building Department with the following,
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and wursual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 farm).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Swom statement from plumber certifying that the solder used in system contains less than 2/10 of 1 % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to Apri19, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Mr. Fish:
The: U. S. Past Office has construction pending under a building permit: at my building an
Village Lane in Orient. I purchased the property in May 1998 and have been planning to
re-open the remaining portionof the front commercial area (approx. 12 ft. x 24 ft. total) along
side the Post OfFice) for the Ice Cream Parlor again for the :next season. The renovations by the
Post Office:put my project temporarily on hold. I am not sure if a building permit would be
required before we re-connect the two antique counters for service area and wall shelving unit
securely in this area,=but am enclosing a building permit (amendment}applteatian far the same
and blank check payable to the Town of Southold for any fee due. There are`no structural
changes to the existing fronYCOmmercial area requested.
Please let me know if any additional permit fee is required for. the minor renovatons and
use of the Ice Cream Parlor area which remains::Shauld abuildingpermit not:be necessary,
please let me know whether any other documentation will be necessary before we re-open.

Dear Mr. Verity:
Mr. Garrett Strang has now prepared the plans for the reopening of the Ice Cream
Parlor at Orient. Three sets are enclosed as requested by you. The size of the ice cream parlor
has been reduced to about 288 sq. ft., which is much smaller than its original size.
I appreciate your assistance, and am requesting a building permit. We would like to
open in time for the warm season approaching very soon. Thank you for all your assistance.
