This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildiugs, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3, Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
I, Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Mr. Kosmynka:
At your written request dated May 14,2008, Planning Department Staff performed a final site
inspection of the entire site build out.
In reference to the above, the Planning Board held a work session on May 27,2008 to discuss the
approved site plan prepared by Kenneth H. Beckman, L.S. dated December 2001, and last
revised August 7, 2007, 2006, the resolution dated June 12,2007, the as-built survey last revised
May 15, 2008 and staff site visit comments regarding the above referenced site plan. In
reference to the above, the Plarming Board has determined the site plan requirements have been
satisfied and include the following items:
I. The Planning Board accepts the as-built survey revised May 15,2008 for the building
location as shown noting all the site changes.
2. The Planning Board accepts the configuration changes of some doors, windows and the
decorative band as built with the Building Department approval.
3. The Planning Board accepts the addition ofa IO'x 40' gravel area in side the fenced site
at the location ofthe fire access gate on the south east side of the property.

4. The Planning Board accepts the changes of the eight parking space enlargements in the
southwest side of the parking lot as built for temporary parking of trucks.
5. The Planning Board accepts the changes of the handicap parking spaces as built at the
site. All handicap parking stalls, access aisles and signage must comply with New
York State Code and ADA requirements.
6. The outdoor lighting appears to meet town code requirements and be shielded so that the
light source is not visible from adjacent properties and roadways.
7. The Planning Board approves the site with the owner's agreement to install the final
downspouts as soon as the new material is received.
Any future changes to the as-built site plan referenced herein must be approved by the Planning
Board.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspecwr may authorize, in writing, th~ extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

WHEREAS, on March 22, 2007, Suffolk County Department of Public Works responded after review
and attached a copy of their previous correspondence, dated January 17, 2006, stating a permit was
not necessary; and
WHEREAS, on March 22, 2007, the Architectural Review Committee reviewed the architectural
drawings and made recommendations and the Planning Board accepted these recommendations at
their Work Session on April 16, 2007; and
WHEREAS, on April 9, 2007, the Southold Town Planning Board, acting under the State
Environmental Quality Review Act (6 NYCRR), Part 617.5 c, initiated the SEQR lead agency
coordination process for this Unlisted Action; and
WHEREAS, on April 11 , 2007, the Suffolk County Department of Planning responded after review
stating, 'The staff has examined the proposed action entitled "North Fork Self Storage" bearing the
stamped date of March 16,2006. Comparing this action with the one that was previously referred to
the Commission, the staff did not find any substantial difference between the town actions. It is the
policy of the Commission not to review any action that it has previously considered unless there
remains substantial differences" and the Planning Board accepts this response; and
WHEREAS, on April 11 ,2007, Suffolk County Department of Health Services responded after review
stating that an application has not been received to date and encouraging the following land use
measures:
1. Maximum use of native species for landscaping purposes.
2. Minimal use of fertilizer-dependant turf and landscaping.
3. Employment of stormwater runoff control measures necessary to maintain runoff on-site; and
WHEREAS, on April 30, 2007, Suffolk County Department of Health Services responded to the
Planning Board's SEQRA coordination, dated April 10, 2007, stating the application, reference #C10-
05-0010, was issued final approval on October 27,2006; and
WHEREAS, on May 2,2007, the Cutchogue Fire District responded after review stating "the area is
adequately protected and will not require any additional water or vehicle access for the site" and the
Planning Board accepts this recommendation; and
WHEREAS, on May 7,2007, the Southold Town Building Inspector reviewed and responded 'The
proposed use Public Warehousing is a permitted use in this LI District and is so certified"; and
WHEREAS, on May 14, 2007, the Southold Town Planning Board, acting under the State
Environmental Quality Review Act, pursuant to 6 NYCRR Part 617.7, performed a coordinated review
of this Unlisted Action. The Planning Board established itself as lead agency, and as lead agency,
made a determination of non-significance and granted a Negative Declaration; and
WHEREAS, on May 17, 2007, the Southold Town Engineer reviewed the site plan materials and
replied stating the adjusted drainage calculations are satisfactory and building heights and
separations should be reviewed by the Building Department and the Planning Board accepts these
comments; and

WHEREAS, the Southold Town Planning Board, pursuant to Chapter 55, Notice of Public Hearing,
has received affidavits that the applicant has complied with the notification provisions; and
WHEREAS, the following items shall be required:
1. All outdoor lighting shall be shielded so that the light source is not visible from adjacent
properties and roadways. Lighting fixtures shall focus and direct the light in such a manner
as to contain the light and glare within property boundaries. The lighting must meet Town
Code requirements.
2. All signs shall meet Southold Town Zoning Codes and shall be subject to approval of the
Southold Town Building Inspector.
3. As per the Landscape Survivability Guarantee, the applicant agrees to replace any of the
landscaping which dies within three (3) years of planting; be it therefore
RESOLVED, that the Southold Town Planning Board has reviewed the proposed action under the
policies of the Town of Southold Local Waterfront Revitalization Program and has determined that the
action is consistent as outlined in the April 18, 2007 memo prepared by the LWRP Coordinator; and
be it further
RESOLVED, that pursuant to Southold Town Code 280-131 Part I, the applicant agrees to
incorporate all the requirements, comments, and recommendations of each reviewing agency as
referenced above and as indicated on the site plan and corresponding attachments; and be it further
RESOLVED, that the Southold Town Planning Board grant approval on the site plan sheet prepared
and certified by Kenneth H. Beckman, L.S., dated December 2001 and last revised June 7, 2007,
Architectural Drawings A-1, A-2 and A-3 prepared and certified by David A. DiGiovanni of DiGiovanni
& Associates, dated May 30, 2007, and authorize the Chairperson to endorse the site plans with the
following conditions:
1. If necessary, the owner, agent and/or applicant shall receive approval from the Suffolk
County Department of Health Services for the approved construction and submit such
approval to the Southold Town Planning Department for review. If such approval varies
from this approved site plan, the Planning Board reserves the right to review a new site
plan application. A copy of the Suffolk County Department of Health Services approved
plan must be submitted to the Southold Town Planning Department within thirty (30) days
of receipt. If the applicant/agent/owner fails to adhere to this requirement, this approval
shall become null and void.
2. If necessary, the applicant/agent/owner must obtain a Phase II State Pollutant Discharge
Elimination System (SPDES) Permit from the New York State Department of Environmental
Conservation (NYSDEC) for the approved construction. A copy of the NYSDEC SPDES
Permit must be submitted to the Southold Town Planning Department within thirty (30) days
of receipt.

3. The owner, agent and/or applicant must ensure all necessary safety precautions have been
provided for during any construction at the site for anyone utilizing the storage facility.
4. The site plan approval requires that all work proposed on the site plan shall be completed
within three (3) years from the date of this resolution.
5. Prior to the request for the Certificate of Occupancy, the owner or authorized agent must
request, in writing, the said Building Inspector and the Planning Board to perform an on-site
inspection to find the site improvements are in conformity with the approved amended site
plan.
6. As per the letter from the Planning Board, dated May 7, 2007, final site plan satisfaction for
this amended site plan will not be issued until the final asphalt base for the entire site has
been completed and an inspection has taken place.
7. Prior to the request for an on-site inspection, the applicanUagenUowner must submit a copy
of all required approvals from any necessary agencies to the Southold Town Planning
Department.
8. If the as-built site improvements vary from the approved site plan, the Planning Board
reserves the right to request a certified as-built site plan detailing all the changes.
9. Any changes from the approved site plan shall require Planning Board approval, and any
such changes without Planning Board approval will be subject to referral to the Town
Attorney's Office for possible legal action.
10. The Planning Board will issue a final site plan approval in the form of a letter following a site
inspection and at the time the site improvements are found to be in conformance with the
approved site plan.
A copy of the approved site plan is enclosed for your records. As per the Town Code, one copy each
will be distributed to the Building Department and Town Engineer/Highway Department.

The Southold Town Planning Board, at a meeting held on Monday, June 11,2007, adopted the
following resolutions:
The final public hearing was closed.
WHEREAS, on March 13,2006, the Planning Board approved a site plan for a mini self-storage
facility with 5 buildings of 37,925 square feet; and
WHEREAS, on March 6, 2007, the Planning Board issued a site plan satisfaction letter for 2 of the 5
buildings, a 5,425 square foot building and a 6,900 square foot building, as shown on the site plan
approved on March 13, 2006; and
WHEREAS, on May 7, 2007, the Planning Board issued a site plan satisfaction letter for a third
14,400 square foot building as shown on the site plan approved on March 13,2006; and
WHEREAS, the agent, Martin Kosmynka, submitted a formal amended site plan for approval on
March 8, 2007; and
WHEREAS, this amended site plan is for the proposed construction of a 40,050 sq. ft. two-story
storage building where the basement is 13,350 sq. ft. and the remaining building is 26,700 sq. ft. and
replaces two one-story buildings totaling 11,200 sq. ft. which were previously approved on March 13,
2006, all on a 3.06-acre parcel in the LI Zone located approximately 370' e/o Depot Lane on the n/s/o

Dear Mr. Kosmynka:
The Planning Board, at their work session on Monday, October 22,2007, reviewed your requests, date
received October 15, 2007 and October 26,2007 (both attached), and approves the following changes to the
approved site plan:
. Convert the existing 15' wide fire gate along Commerce Drive to a permanent operating gate, to be
used as an exit from the storage facility. The Planning Board accepts the October 29, 2007 letter
from Cutchogue Fire District endorsing this change. The Planning Board approves this driveway
opening provided that:
o The 15' wide gate would operate in the same manner as the other existing 2 gates, i.e. open
automatically should the fire alarm engage to allow additional access to the site for the fire
department.
o The direction of the southwestern travel lane must be revised to reflect the requested
change. Once construction is complete, a revised as- built showing this correction must be
submitted to this office for review.
. The HV AC compressors for the approved 40,050 sq. ft. building may be located within the existing
eastern 5' buffer and northern 4' buffer, screened from view by vegetation (arborvitaes). The
Planning Board approves this change provided that:
o The location of all HV AC compressors must be solely located on the subject parcel only.
The compressors may not be placed on any neighboring property or share boundaty lines
with any neighboring properties.
o The existing vegetation suitably conceals the compressors. The Planning Board reserves the
right to require additional landscaping if deemed necessaty.
The Planning Board recognizes this proposed change will result in the reduction of two (2) parking spaces,
however, the total parking (actual and landbanked) provided exceeds the minimum requirements.

satisfaction. In addition, please submit three (3) copies of an as-built site plan depicting all changes onsite.

GENERAL
The Fire-Ute Alarms 50355 and 50355T addressable,
low-profile plug-in photoelectric detectors use a state-of-
the-art photoelectric sensing chamber with communica~
tions to provide open area protection and are used exclu-
sively with Fire.Ute's M5-9200, M5-9200UO, and M5-9600
Addressable Fire Alarm Control Panels (FACPs). The
SD355T adds thermal sensors that will alarm at a fixed
temperature of 135"F (57"C). Since these detectors are
addressable, they will help emergency personnel quickly
locate a fire during its early stages, potentially saving pre-
cious rescue time while also reducing property damage.
Two LEDs on each sensor light to provide a local, visible
sensor indication. Remote LED annunciator capability is
available as an optional accessory (PIN RA400Z).

CONSTRUCTION
These detectors are constructed of off-white LEXAN@.
SD355(T) plug-in, low-profile smoke detectors are designed
to commercial standards and offer an attractive appear-
ance.
LEXAN@is a registered trademark of GE Plastics, a subsidiary of
General Electric Company.
INSTALLATION
SD350(T) plug-in detectors use a detachable mounting base
to simplify installation, service and maintenance. Mount
base on box which is at least 1.5 inches (3.81 em) deep.
Suitable boxes include:
4.0" (10.16 em) square box with plaster ring.
4.0" (10.16 em) octagonal box.
3.5" (8.89 em) octagonal box.
Single-gang box.
NOTE: Because of the inherent supervision provided by the SLC
loop, end-or-line resistors are not required. Wiring ~T-t8pS~ or
branches are permitted for Style 4 (Class B) wiring only.
OPERATION
Each SD355(T) uses one of 99 possible addresses on the
MS-9200 and MS-9200UD Signaling Line Circuit (SLC), or
up to 318 (159 on each loop) on the MS-9600 SLC. It re-
sponds to regular polls from the system and reports its type
and status.
The 8D355(T) addressable photoelectric sensor's unique
unipolar chamber responds quickly and uniformly to a broad
range of smoke conditions and can withstand wind gusts
up to 1,500 feet-per-minute (7.6 m/sec.) without sending
an alarm level signal. Because of its unipolar chamber, the
SD355(T) is approximately two times more responsive than
most photoelectric sensors. This makes it a more stable
detector.
DETECTOR SENSITIVITY TEST
Each detector can have their sensitivity tested (required
per NFPA 72, Chapter 7 on Inspection, Testing and Main-
tenance) when installed/connected to an MS-9200, MS-
9200UD, or MS-9600 addressable fire alarm control panel.
The results of the sensitivity test can be printed off the MS-
9200, MS-9200UD, or MS-9600 for record keeping.

GENERAL
The Wheelock Series NS Horn Strobe Appliances will
satisfy virtually all requirements for indoor, wall mount
applications.
The Series N H Ham and the horn portion of the Series
NS include a selectable continuous horn tone or
temporal pattern (Code 3) with selectable dBA settings
of 90 or 95 dBA.
Strobe options include 157500 or Wheelock's patented

GENERAL NOTES
Strobes are designed to flash at 1 flash per second minimum
over their "Regulated Voltage Range". Note that NFPA-72 speci-
fies a flash rate of 1 to 2 flashes per second and ADA Guide-
lines specify a flash rate of 1 to 3 flashes per second.
All candela ratings represent minimum effective Strobe inten-
sity based on UL Standard 1971.
Series NS Strobe pmducts are listed under UL Standard 1971
for indoor use with a temperature range of 320F to 1200F (DOC

ARCHITECTS AND ENGINEERS SPECIFICATIONS
The audiblelvisual notification appliances shall be Wheelock Series NS Ham Strobe appliances and Series NH Ham
appliances or approved equals. The Series NS appliances shall meet and be listed for UL Standard 1971 (Emergency
Devices for the Hearing-Impaired for Indoor Fire Protection Service). The Series NH Horn shall be UL Listed under
Standard 464 (Fire Protective Signaling). The horn strobe shall be listed for indoor use and shall meet the require-
ments of FCC Part 15 Class B. All inputs shall be compatible with standard reverse polarity supervision of circuit wiring
by the Fire Alarm Control Panel (FACP).
The audible portion o!the appliance shall have a minimum of two (2) field selectable settings for dBA levels (90 and 95 dBA)
and shall have a choice of continuous or temporal (Code 3) audible outputs.
The strobe portion o!the appliance shall produce a flash rate of one (1) flash per second over the Regulated Voltage Range
and shall incorporate a Xenon f1ashtube enclosed in a rugged Lexan lens. The Series NS shall be of low current design.
Where wall mount, Multi-Candela appliances are specified, the strobe intensity shall have field selectable settings and shall
be rated per UL Standard 1971 for 15/30/75/110 candela. The selector switch for selecting the candela setting shall be
tamper resistant. The 1575 candela strobe shall be specified when 15 candela UL Standard 1971 Listing with 75 candela on-
axis is required (e.g. ADA compliance).
When synchronization is required, the appliance shall be compatible with Wheelock's SM, DSM Sync Modules or a Power
Supply with Wheelock's built-in Patented Sync Protocol. The strobes shalf not drift out of synchronization at any time during
operation. If the sync module or Power Supply fails to operate] (i.e., contacts remain closed), the strobes shall revert to a
non-synchronized flash-rate. The appliance shall also be designed so that the audible signal may be silenced while maintain-
ing strobe activation.
The Series NS Horn Strobes and NH horn shall incorporate a Patented Universal Mounting Plate that shall allow mounting to
a single-gang, double-gang, 4-inch square, 1 OOmm European type backboxes, or the SHBB Surface Backbox. If required, an
NATP (Notification Appliance Trimplate) shall be provided.
All notification appliances shall be backward compatible.

GENERAL
The Fire'Llle Alarms BG-12LX is a state-of-the-art, dual-
action (i.e., requires two motions to activate the station)
pull station that includes an addressable interface
(mounted inside) for Fire.Ute's addressable MS-9200, MS-
9200UD, and MS-9600 fire alarm control panels. Because
the BG-12LX is addressable, the control panel can dis-
play the exact location of the activated manual pull station.
This leads fire service personnel quickly to the location of
the alarm.

CONSTRUCTION
Shell. door. and handle are molded of durabie LEXAN@
(or polycarbonate equivalent) with a textured finish.
OPERATION
Pushing in, then pulling down on the handle causes it to
latch in the down/activated position. Once latched, the word
"ACTIVATED" (in bright yellow) appears at the top of the
handle. while a portion of the handle protrudes from the
bottom of the station. To reset the station, simply unlock
the station with the key and pull the door open. This action
resets the handle; closing the door automatically resets
the switch.

INSTALLATION
The BG-12LX can be semi-flush mounted into a single-
gang, double-gang, or standard 4" (10.16 em) square elec-
lrical outlet box, or surface mounted to the Model 86-1/0 or
86-10 surface backbox. If the 6G-12LX is semi-flush
mounted, then the optional trim ring (6G- TR) may be used.
The BG-TR is usually needed for semi-flush mounting with
4" (10.16 em) or double-gang boxes (not with single-gang
boxes).

Manual Fire Alarm Stations shall be non-code, with a key-
operated reset lock in order that they may be tested, and
so designed that after actual Emergency Operation, they
cannot be restored to normal except by use of a key. An
operated station shall automatically condition itself so as
to be visually detected as activated. Manual stations shall
be constructed of red-colored LEXAN@ (or polycarbonate
equivalent) with clearly visible operating instructions pro-
vided on the cover. The word FIRE shall appear on the
front of the stations in white ietters, 1.00 inches (2.54 em)
or larger. Stations shall be suitable for surface mounting
on matching backbox 88-1/0 or 88-10; or semi-flush mount-
ing on a standard single-gang, double-gang, or 4" (10.16
em) square electrical box, and shall be installed within the
limits defined by the Americans with Disabilities Act (ADA)
or per national/local requirements. Manual Stations shall
be Underwriters laboratories listed.
Manual stations shall connect with two wires to one of the
control panel SlC loops. The manual station shall. on com-
mand from the control panel, send data to the panel repre-
senting the state of the manual switch. Manual stations shall
provide address setting by use of rotary decimal switches.

The Fire-Lite MS-9050UD is a Fire Alarm Control Panel
(FACP) and Digital Alarm CommunicatorlTransmilter (DACT)
combined into one circuit board. This compact, cost effective,
intelligent addressable control panel supports up to 50 addres-
sable devices of any type of detectors and modules. With an
extensive list of powerful features, the MS-9050UD programs
just like Fire-Ute's larger products. yet fits into applications pre-
viously served only by conventional panels.
The MS-90SOUO's integral DACT transmits system status
(alarms, troubles, AC loss. etc.) to a Central Station via the
public switched telephone network.
The power supply and all electronics are contained on a single

Wiring Requirements
While shielded wire is not required, it is recommended that all SlC wiring be twisted-pair to minimize the effects of electrical inter-
ference. Wire size should be no smaller than 18 AWG (0.78 mm') and no larger than 12 AWG (3.1 mm'). The wire size depends
on the length of the SLC circuit. Use the following table to determine the specific wiring requirements for the SLC. (Refer to the
panel manual for complete details.)

General
The ANN-50 Annunciator is a compact, backlit, aD-character
LCD fire annunciator that mimics the Fire Alarm Control Panel
(FACP) display. It provides system status indicators for AC
Power, Alarm. Trouble. Supervisory, and Alarm Silenced condi-
tions. The ANN-80 and the FACP communicate over a two-wire
serial interface employ;ng the ANN-BUS communication format.
Connected devices are powered. via two additional wires, by
either the host FACP or a remote UL-Iisted. filtered power supply.
The ANN-80 displays English-language text of system point
information including device type, zone, independent point
alarm. trouble or supervisory status, as well as any custom
alpha labels programmed into the control panel. It includes con-
trol switches for remote control of critical system functions. (A
keyswitch prevents unauthorized operation of the control
switches.)
Up to eight ANN-80s may be connected to the ANN-Bus of each
FACP. No programming is required, which saves time during
system commissioning.

The ANN-BUS can be powered by an auxiliary power supply
when the maximum number of ANN-BUS devices exceeds the
ANN-BUS power requirements. See the FACP manual for more
information.

ANN-BUS DEVICE ADDRESSING
Each ANN-BUS device requires a unique address (10 Number)
in order to communicate with the FACP. A maximum of 8 devices
can be connected to the FAGP ANN-BUS communication circuit.
See the FAC? manual for more information.
WIRE REQUIREMENTS: COMMUNICA TIONS CIRCUIT
The ANN.80 connects to the FACP ANN-BUS communications
circuit. To determine the type of wire and the maximum wiring
distance that can be used with FACP ANN-BUS accessory mod-
ules, it is necessary to calculate the total worst case current
draw for all modules on a single 4-conductor bus. The total worst
case current draw is calculated by adding the individual worst
case currents for each module.
NOTE: For total worst case current draw on a single ANN-BUS
refer to appropriate FACP manual.
After calculating the total worst case current draw, the following
table specifies the maximum distance the modules can be
located from the FACP on a single wire run. The table ensures
6.0 volts of line drop maximum. In general, the wire length Is lim-
ited by resistance, but for heavier wire gauges, capacitance Is
the limiting factor.
These cases are marked in the chart with an asterisk (*). Maxi~
mum length can never be more than 6,000 feet (1,800 m),
regardless of gauge used. See table below.
WIRE REQUIREMENTS: POWER CIRCUIT
14 to 18 AWG (0.75 - 2.08 mm') wire for 24 VDC power cir-
cuit is acceptabJe. Power wire distance limitation is set by 1.2
volt maximum line drop form source to end of circuit.
AU connections are power-limited and supervised.
A maximum of eight ANN-BO modules may be connected to
this circuit.
