This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9,1957) non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

INSTRUCTIONS
, a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
set's of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location oflot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of a Building Permit pursuant to the
Building Zone Ordinance of the Town of South old, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The
applicant agrees to comply with all applicable laws, ordinances, building code, housing code, and regulations, and to admit
authorized inspectors on premises and in building for necessary inspections.

Dear Mr. Damianos:
Thank you for the cooperation during the fire inspection of Pindar Winery on June 1,
2004.
The maximum occupancy of your establishment is 136. This figure is shown on the
enclosed occupancy certificate. This certificate supercedes all previous certificates and
is valid for one year from the indicated date. If the occupancy indicated is 50 or higher
this certificate shall be posted in a conspicuous location near the main entrance to your
establishment. The additional certificate shall be posted in the tasting room.
New and existing buildings shall have building numbers placed in a position to be
plainly legible from the street or road fronting the property. The numbers shall be a
minimum of 4 inches high. Your building number is 37295.
During the inspection the following violations were noted and should be corrected
immediately.
Winery
The fire alarm and life safety systems shall be maintained in an operable condition at all
times. The fire and life safety systems shall be properly inspected and tested annually.
A written record shall be maintained and shall be made available to the code
enforcement official for a minimum of 2 years.

The unused components of the old alarm system shall be removed. If alarm
components are installed the parts must function properly.
All emergency lights must be in proper operating condition at all times. One set in the
tasting room did not work properly.
Open junction boxes and Open-wiring splices shall be prohibited. Approved covers
shall be on all switch and electrical outlet boxes. There were open boxes on the first
floor and in the basement.
The building shall be properly placarded for hazardous materials. Maximum storage of
sulfur dioxide is 810 cubic feet. Each gas tank contains between 300 and 330 cubic
feet. The numbers on the placard should be Health 3, Fire 0, reactivity 0, and no water
used. See enclosed information. In the event that the stored amount exceeds 810
cubic feet, the storage area will be deemed a hazardous materials storage area and will
be required to be separated, protected, and alarmed.
The liquid propane tanks shall be stored outside or in another location.
Fire extinguishers shall be inspected monthly by a designated employee and inspected
and tagged yearly by an approved servicing company. The extinguisher in the office
was discharged and another extinguisher was not properly tagged.
A handrail is necessary on the other side of the stairs to the basement.
Storaqe Buildinq
Fire extinguishers shall not be obstructed or obscured from view. In rooms or areas in
which visual obstruction cannot be completely avoided, means shall be provided to
indicate the locations of extinguishers. More fire extinguishers are required.
Extinguishers shall be located so that travel distance to an extinguisher is less than 75
feet.

Certification of non-flammability for the vapor barrier is required.
In addition the following are necessary for a Certificate of occupancy.
1. The fire area or areas need to be certified or corrected. The fire area is too
large.
2. The building uses need to be certified as they relate to the building construction
and occupancy separations.
3. As built plans are required.
4. The building and plans shall be reviewed to be sure that both adhere to the
applicable state and local codes.
5. Final building inspection.
6. Final fire inspection.

At the written request of Alexander Damianos, dated January 17, 2007, requesting final site plan inspection,
Planning Department staff perfonned a site visit on January 19, 2007.
In reference to the above, the Planning Board held a work session on January 22,2007 to discuss the
approved site plan prepared by Clarles W. Southard dated May 12, 2001 and last revised January 3, 2007,
the resolution dated January 9, 2007 and staff site visit conunents regarding the above referenced site plan.
In reference to the above, the Planning Board has detennined the site plan requirements have been satisfied
and include the following items:
· All handicap parking stalls, access aisles and signage must comply with New York State Gxle and
ADA requirements.
This letter does not condone any changes from the as-built site plan and approvals from other agencies if
required.

Rate-Anticipation Heat Detectors
Section: Conventional Initiating Devices
GENERAL
The Thermotech 302 Series rate-anticipation heat detec-
tors operate within a controlled range of two to three de-
grees of their set points, regardless of the speed or rate of
temperature rise. These detectors are available in either
1350F or 1940F ratings.
The 302 Series are normally-open devices designed es-
pecially for fire detection and alarm systems.

FEATURES
Immediate response. The 302 Series activate
whenever ambient air temperature reaches a
detector's setting, eliminating the thermal time lag
inherent in conventional heat detectors.
Eliminates false alarms. The 302 Series do not
respond to momentary temperature fluctuations

PRINCIPLES OF OPERATION
The 302 Series rate-anticipation heat detectors re-
spond and activate the fire alarm immediately when-
ever the ambient temperature reaches the preset
temperature setting. Under rapid heat rise condi-
tions, the rate-anticipation feature enables the de-
tector to respond one to three degrees ahead of
the setting. At the same time, however, it does not
respond to momentary temperature fluctuations be-
low the selected protection level, thus eliminating
false alarms. When temperature drops back down
below the protection level, the detector automati-
cally resets itself.

302 Series detectors have a smooth.ceiling UL rating of
50' x 50' (15.24 x 15.24 meters) and are the only type of
heat detectors having such a rating on both fixed tempera-
ture and rate anticipation. The 302-H is designed for hori-
zontal mounting and UL rated for 40' x 40' (12.19 x 12.19
meters).

The FMM-462 Series Addressable Manual Stations allow
people evacuating a building to initiate an alarm and
alert others still inside. This UL-Iisted initiating device is
compatible with the Bosch G Series Control Panels and
the D9124 Fire Alarm Control Panel (FACP).
The POPIT module is addressed using on-board slide
switches. It is then connected to a D9124 data
expansion loop. The POPIT is polled by the control panel
and sends reports on any opens, shorts, or ground faults
on the manual station's wiring. The addressable feature
allows the agencies responding to alarm or trouble
conditions to go to the point that initiated the alarm or
trouble report.
These low-profile manual stations are die cast with
rounded edges to fit most design applications. All
metallic components are painted or plated to inhibit
corrosion. The FMM-462 Series Manual Stations
conform with the Americans with Disabilities Act (ADA)
standards.

tures that include the widest range of candela options available and the capability to
recognize and self-adjust for either 12 or 24 volt operation. With an overall feature
set that combines performance, installation ease, flexibility, and a consistent,
aesthetically pleasing appearance. the SpectrAlert Selectable Output devices
provide both the innovation and efficiency synonymous with the SpectrAlert name.
Performance. SpectrAlert selectable output wall-mount horns, strobes, and horn/strobes
offer key performance features long associated with the SpectrAlert name. The select-
able candela strobes and horn/strobes offer average current draws that are not only
lower than conventional fixed-candela SpectrAlert products, but al~o lower than similar
selectable candela products. By consuming less current, the ability to connect even more
devices per loop is possible, resulting in a lower installed cost.
Installation. SpectrAlert selectable output horns, strobes, and horn/strobes offer the
same installation-friendly features synonymous with the SpectrAlert name, such as the
option of 2- and 4-wire operation; the ability to use standard size backboxes with no
encroachment into the box; and universal mounting incorporating the labor-saving
QuickClick.... feature. Such labor-savings features make wire connections simple and fast,
further reducing installed cost.
Flexibility. SpectrAlert selectable output strobes and horn/strobes offer the broadest
range of candela options. In addition, the selectable output strobes and horn/strobes
can operate on either 12V or 24V, with no setting required; the device recognizes and
self-adjusts to the correct current automatically. Temporal 3 or Continuous tone options
continue to be available, in either an Electromechanical or 3kHz pattern.
Aesthetics. SpectrAlert selectable output horns, strobes, and horn/strobes incorporate
the same stylish, low profile design of the conventional SpectrAlert products, for a con-
sistent and aesthetically pleasing appearance across the entire product line.
Engineering Specifications
General
SpectrAlert horns, strobes and horn/strobes shall be capable of
mounting to a standard 4" x 4" x 11/2" back box or a single gang
2" x 4" x F!~" back box using the universal mounting plate
included with each SpectrAlert product. Also, SpectrAlert prod-
ucts, when used in conjunction with the accessory Sync-Circuit
Module, shall be powered from a non-coded power supply
and shall operate on 12 or 24 volts. 12 volt rated devices shall
have an operating voltage range of 9-17.5 volts. 24-volt rated
devices shall have an operating voltage range or 17-33 volts.
SpectrAlert products shall have an operating temperature of
3ZO to 1200F and operate from a regulated DC or full wave
rectified, unfiltered power supply.
Strobe
Strobe shall be a System Sensor SpectrAlert Model_
listed to UL 1971 and be approved for fire protective service. The
strobe shall be wired as a primary signaling notification appliance
and comply with the Americans with Disabilities Act require-
ments for visible signaling appliances, flashing at 1Hz over the
strobe's entire operating voltage range. The strobe light shall con-
sist of a xenon flash tube and associated lenslreflector system.

The D7024 Addressable Fire Alarm Control Panels (FACPs)
can be used in commercial and public building applications
such as schools, universities. manufacturing plants, and health
care facilities. They are listed by UL for central station,local,
auxiliary, and remote station systems.
Adding the D7039 Multiplex Expansion Modules to the
D7024 Conventional FACPs with version 2.02 or higher
firmware makes the FACPs addressable with up to 255
addressable points. The FACPs support both two-wire and
four-wire detectors. Each FACP has a built-in digital fire alarm
communicator transmitter (DACT) and four on-board
detection zones that can be expanded to eight zones. Each
wne can support up to 20 two-wire detectors, or any number
of four-wire detectors (depending on available power).

Two notification appliance circuits (NAC) provide up to 4 A of
24 VDC power to operate horns, strobes, bells, and other
notification appliances. Each NAC can be programmed to

A viewing port in the keyed control panel enclosure door
leaves the system status light-emitting diodes (LEDs) and
liquid-crystal display (LCD) visible while maintaining system
keypad security. Unlocking the enclosure door provides access
to the system keys for silencing alarm and trouble conditions,
zone bypassing, detector resetting, testing, other fire
functions, and programming. These functions are also
available on all D7033 LCD Keypads connected to the system.
All D7036 LCD Annunciators provide custom text
annunciation on a 32-character, backlit, two-line LCD display.
All system keypads and annunciators have built-in sounders.
Back-light intensity and sounder volume are programmable.
Personal Identification Numbers (PINs)
There are 100 PINs available for accessing system control.
Assign each number an authority level to limit the system
control functions that can be accessed.

The system stores events in a 499-event history buffer.
Central Station Reporting
The D7024 FACP sends reports to two telephone numbers
with full single, double, and backup reporting and
communicate in BFSK, SIA, Contact!D, Modem llIa 2, and
4/2 and 3/1 Tone Bursts.

The D7024 FACP recognizes the CleanMe signal from Bosch's
F220 Smoke Detectors. If a detector's smoke chamber is dirty,
the detector sends a signal on the loop (two-wire or
addressable) and the control panel annunciates that there is a
dirty detector on the loop.

System Sensor Model BEAM1224 is a 4-wire conventional projected beam smoke
detector. It is uniquely suited for protecting open areas with high ceilings where other
methods of smoke detection are difficult to install and maintain. It is to be used with
UL Listed compatible control panels only. Installation of the single-ended reflective
design is much easier than the dual-ended projected beam detectors. Alignment is
quickly accomplished via an optical sight and a 2-digit signal strength meter incor-
porated into the product. Listed for operation from _220P to 1310F, BEAM1224 can
be used in open area applications to provide early warning in environments where
temperature extremes exceed the capability of other types of smoke detection.
BEAM1224 consists of a transmitter/receiver unit and a reflector. When smoke enters
the area between the unit and the reflector it causes a reduction in the signal and,
when the smoke level reaches the predetermined threshold, an alarm is activated.
BEAM1224 has four standard sensitivity selections along with two Acclimate set-
tings. When either of the two Acclimate settings are selected the detector will auto-
matically adjust its sensitivity using advanced software algorithms to select the opti-
mum sensitivity for the specific environment.
BEAM1224S is equipped with an integral sensitivity test feature that consists of a
test filter attached to a servo motor inside the detector optics. Using the remote test
station RTS451, the motor is activated and moves the filter in the pathway of the
light beam, thereby testing detector sensitivity. This integral sensitivity test feature
allows the user to quickly and easily meet the annual maintenance and test require-
ments of NFPA 72.
