This application must be filled in by typewriter or ink and submitted Co the Building Department with the following:
A. For new building or new use:
1. Final swrvey of property with accurate location of all buildings, property lines, streets, and unusual natural or
lopographic features.
2. Final Approval from IIealth Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Plamiing Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and °°pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed app]ication and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

The original existing structure was inspected by me in the middle of the year 2000. I notified the
owners that the structure then existing was sub-standard in that much of the framing was
undersized, there was no insulation, and various improper materials (i.e. Celotex fiber board; in
lieu of gypsum wall board) had been used during the original construction. The owners
commenced interior renovation work and during this work the structure was damaged by a falling
tree requiring replacement of much of the roof structure. At that time, the owners undertook a
complete renovation of the structure as shown on the enclosed plans.
I have inspected the construction since its completion and it appears that all work has been done to
code with the exception of the reinforcing of the first floor framing which has not been done to
