a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. fee according to schedule.
b. Plot plan showing location oflot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of a Building Permit pursuant to the
Building Zone Ordinance of the Town of Southold, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The
applicant agrees to comply with all applicable laws, ordinances, building code, housing code, and regulations, and to admit
authorized inspectors on premises and in building for necessary inspections.

TERMS AND CONDITIONS
The Permittee Carol Freda. residing at 7715 Indian Neck Lane. Peconic. NY, as part of the
consideration for the issuance of the Permit does understand and prescribe to the following:
I. That the said Board of Trustees and the Town of Southold are released from any and all
damages, or claims for damages, of suits arising directly or indirectly as a result of any
operation performed pursuant to this permit, and the said Permittee will, at his or her own
expense, defend any and all such suits initiated by third parties, and the said Permittee
assumes full liability with respect thereto, to the complete exclusion of the Board of
Trustees of the Town of Southold.
2. That this Permit is valid for a period of 24 months, which is considered to be the estimated
time required to complete the work involved, but should circumstances warrant, request for
an extension may be made to the Board at a later date.
3. That this Permit should be retained indefinitely, or as long as the said Permittee wishes to
maintain the structure or project involved, to provide evidence to anyone concerned that
authorization was originally obtained.
4. That the work involved will be subject to the inspection and approval of the Board or its
agents, and non-compliance with the provisions of the originating application may be cause
for revocation of this Permit by resolution of the said Board.
5. That there will be no unreasonable interference with navigation as a result ofthe work
herein authorized.
6. That there shall be no interference with the right of the public to pass and repass along the
beach between high and low water marks.
7. That if future operations of the Town of South old require the removal and/or alterations in
the location of the work herein authorized, or if, in the opinion of the Board of Trustees,
the work shall cause unreasonable obstruction to free navigation, the said Permittee will be
required, upon due notice, to remove or alter this work project herein stated without
expenses to the Town of Southold.
8. The Permittee is required to provide evidence that a copy of this Trustee permit has been
recorded with the Suffolk County Department of Real Properties Office as a notice
covenant and deed restriction to the deed of the subject parcel. Such evidence shall be
provided within ninety (90) calendar days of issuance of this permit.
9. That the said Board will be notified by the Permittee of the completion of the work
authorized.
10. That the Permittee will obtain all other permits and consents that may be required
supplemental to this permit, which may be subject to revoke upon failure to obtain same.

Dear Ms. Mesiano:
The Board of Town Trustees took the following action during its regular meeting held on
Wed., December 13, 2006 regarding the above matter:
WHEREAS, Catherine Mesiano on behalf of CAROL FREDA applied to the Southold
Town Trustees for a permit under the provisions of Chapter 275 of the Southold Town
Code, the Wetland Ordinance of the Town of Southold, application dated November 20,
2006, and,
WHEREAS, said application was referred to the Southold Town Conservation Advisory
Council and to the Local Waterfront Revitalization Program Coordinator for their findings
and recommendations, and,
WHEREAS, the Board of Trustees has furthered Policy 6 of the Local Waterfront
Revitalization Program to the greatest extent possible through the imposition of the
following Best Management Practice requirements: the installation of gutters and
drywells to contain the roof run-off, and a line of staked hay bales during construction
activities, and,
WHEREAS, a Public Hearing was held by the Town Trustees with respect to said
application on December 13, 2006, at which time all interested persons were given an
opportunity to be heard, and,

WHEREAS, the Board members have personally viewed and are familiar with the
premises in question and the surrounding area, and,
WHEREAS, the Board has considered all the testimony and documentation submitted
concerning this application, and,
WHEREAS, the structure complies with the standards set forth in Chapter 275 of the
Southold Town Code,
WHEREAS, the Board has determined that the project as proposed will not affect the
health, safety and general welfare of the people of the town,
NOW THEREFORE BE IT,
RESOLVED, that for the mitigating factors and based upon the Best Management
Practice requirement imposed above, the Board of Trustees deems the action to be
Consistent with the local Waterfront Revitalization Program pursuant to Chapter 268-5
of the Southold Town Code, and,
RESOLVED, that the Board of Trustees approve the application of CAROL FREDA to
remove 14'X 22' wood deck and step, construct 14'X 22' one-story addition it its place;
construct 12.1 'X 7.1' proposed second-story addition over existing screened deck, with
part of deck to be removed; construct 30'X 28' two-story addition; and remove existing
sanitary system and install new sanitary system, with the condition gutters and drywells
are installed to contain the roof run-off and a line of staked hay bales is placed down
prior to any activity and maintained throughout construction, and all as depicted on the
survey prepared by Joseph A. Ingegno dated September 14,2006.
Permit to construct and complete project will expire two years from the date the permit
is signed. Fees must be paid, if applicable, and permit issued within six months of the
date of this notification.
Inspections are required at a fee of $50.00 per inspection. (See attached schedule.)

Dear Ms. Freda:
Based on the information your agent has submitted, the New York State Department of Environmental
Conservation has made the following determination.
Your proposed project to construct additions to the existing single family dwelling and remove the
existing sanitary system and install a new sanitary system, as shown on the survey prepared by Joseph
A. Ingegno last revised on 12/28/06, is landward of the topographic crest of a bank. The crest of the
bank ranges in elevation from 10 feet to 15 feet. Therefore, your project is beyond the jurisdiction of
Article 25 Tidal Wetlands.
In accordance with the current Tidal Wetlands Land Use Regulations (6 NYCRR Part 661) no permit
is required under the Tidal Wetlands Act. Please be advised, however, that no construction,
sedimentation, or disturbance of any kind may take place seaward of the tidal wetlands jurisdictional
boundary, as indicated above, without a permit. It is your responsibility to ensure that all necessary
precautions are taken to prevent any sedimentation or other alteration or disturbance to the ground
surface or vegetation within Tidal Wetlands jurisdiction which may result from your project. Such
precautions may include maintaining adequate work area between the tidal wetland jurisdictional
boundary and your project (i.e. a 15' to 20' wide construction area) or erecting a temporary fence,
barrier, or hay bale berm.
Please be further advised that this letter does not relieve you of the responsibility of obtaining any
necessary permits or approvals from other agencies. /'l

As per your request, I have reviewed the permit application & concluded a drainage
assessment review for the above referenced property. The survey / site plan has been prepared by
the office of Joseph A. Ingegno and has been dated of September 14, 2006. Please consider the
following:
1. The assessment form submitted by the applicant did not indicate the containment or
treatment of a two (2") inch rainfall. Containment of Storm-water is required by the
Town Drainage Code. The Trustee Permit issued to this applicant also requires the
installation of Gutters, Leaders and Drywells to contain storm-water run-off.
2. Drainage Calculations have not been provided, Containment of a two (2") inch rainfall
is required for all development areas where surfaces are hardened. Roof structures
and other impervious surfaces will require a runoff coefficient of one (1). Pervious
gravel driveways require a runoff coefficient of sixty (0.6) percent.
3. Drainage designs should include the size and location of all proposed Leaching Pool
structures. The plans should also show how leader pipe will be connected to the
Leaching Pools. These structures should be labeled and/or numbered to quantify the
total drainage provided. Drainage details of the leaching pool structures should also
be provided.
4. All proposed construction or areas of improvement should be screened or separated
from the wetland areas with hay bales and silt fencing to meet the minimum standards
& requirements of New York State Dept of Environmental Conservation Phase II State
Pollutant Discharge Elimination System (SPDES) Program.
