This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 fonn).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for tlie building,
6. Submit Plarming Board Approval of completed site plan requirements,
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and a consent to inspect signed by the applicant. If a Certificate of Occupancy
is denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Construction Code. ~
2. All electrical work is to conform to local. N.E.C. and Underwriters Require
3. All materials. assemblies. construction and equipment shall conform to the
regulations of the N.Y.s. Uniform Fire Prevention & Building Code and shall
conform to generally accepted standards.
4. Contractor shall furnish all labor and materials to carry out the work shown on the
drawings herein. Contractor shall exclude work shown "Not in Contract" (N.I.C.)
5. Do not scale the drawings. Any discrepancies in the plans or details shall be reported
in writing to the engineer for clarification before commencement of work.
6. Contractor to coordinate all inspections from all agencies and municipalities having
jurisdiction.
7. Contractor and/or respective subcontractors shall be strictly responsible for their
work and shall be responsible for adequately bracing and protecting all work during
construction against damage. breakage. collapse, distortion, and misalignment
according to all applicable codes, standards, and good construction practices.
8. Contractor is to verify all existing conditions prior to start of work. Notify engineer of
any discrepancies.
9. Contractor is solely responsible for maintaining safety on the site. He shall protect
the public and all others from the hazards of excavations and all other damages
during the course of his work.
10. Adequate escape facilities shall be maintained at all times in buildings under
construction.
11. All footings must rest on undisturbed soil having a minimum bearing value of 2 tons
P.s.F.. All concrete block must be load bearing type. All concrete shall be 3000
P,S.1. @ 28 days.
12. All structural framing lumber shall be Douglas fir #2 with a minimum fiber stress of
