This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey ofpropcrty with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation finn Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of 1 % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Mr. Gillen;
In response to your Inspection notice of 5-2-07 on the above referenced premises, and our subsequent
conversation, I submit the following as satisfaction of the noted deficiencies.
1. The nuts missing from the anchor bolts at the perimeter of the garage have been
installed.
2. The insulation in the flat ceiling of the room over the Garage has had an
additional layer ofR-II laid over it bringing the "R" value up to R-41.
3. The insulation in the flat ceiling of the Master Bedroom and Loft Office
has had an additional layer ofR-IIlaid over it bringing the "R" value
up to R-41.
All of the above have been done under my personal supervision and I trust this will serve as a suitable
verification.

Dear Mr. Gillen;
In response to your Inspection notice of 9-26-07 on the above referenced premises, and our subsequent
conversation, I submit the following as Certification ofthe below listed items:
I. The garage and basement laundry dryers were properly vented to the
exterior using rigid metal ductwork.
2. The finished basement is insulated along the exterior perimeter walls
as it is conditioned space. Therefore no insulation is required in the
ceiling although there was insulation installed in the ceiling of the
media room for sound deadening.
3. The Basement exit leads directly to the exterior ofthe house through
the door at the head of the basement stairs as required by code.
The remaining items listed below have been completed and are ready for verification at your next inspection:
1. Handrails at new exterior and interior stairs have been installed.
2. Fire Doors at garage have becn adjusted to elose completely.
3. Basement bathroom door has been adjusted to latch properly.
4. PVC pipe below garage sink has had fire collar installed.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspcctor with 3
scts of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
arcas, and waterways.
c. The work covered by this application may not be commcnced before issuance of Building Pemtit
d. Upon approval of this application, the Building Inspector will issue a Building Pemtit to the applicant Such a pennit
shall bc kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issucs a Ccrtificate of Occupancy.
f Evcry building permit shall expire if the work authorizcd has not commcnccd within 12 months after the date of
issuance or has not been completed within 18 months from such date. Ifno zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance ofa Building Pemtit pursuant to the
Building Zone Ordinance of the Town of Southold, Suffolk County, New York. and other applicable Laws, Ordinances or
Rcgulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The
applicant agrees to comply with all applicable laws, ordinances, building code, housing code, and regulations, and to admit
authorized inspectors on premises and in building for necessary inspections. ~
