2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber celiifying that the solder used in system contains less than 2/1 0 of I % lead.
S. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Dear Tony,
In completion of my contract with you for structural design services on your new home, I made
four visits to the site to observe the progress of construction and evaluate its conformance to the
structural plans and specifications. Those plans and specifications were based upon the
following design codes and standards;
1. Residential Code of New York State
2. ASCE Standard Minimum Design Loads for Buildings and Other
Structures (98)
3. American Forest and Paper Association - Wood Frame Constru!ltion
Manual, High Wind Edition
4. American Institute of Steel Construction
My site visits were undertaken in November, 2005 (at'the completion of the steel frame),
January, 2006 (completion of first floor framing), April, 2006 (completion of roof framing) and
November, 2006 (completion of all framing within the house). Within normal construction
tolerances, all construction conformed to the original plans; there were no field changes made.
Both the steel and wood framing was erected according to the plans and specifications. Steel
connections and reinforcements strictly followed the structural details. Hurricane clips, tie-
downs, strapping and nailing of the wood framing was in accordance with the referenced codes
and 1he project nailing schedule. In my professional opinion the quality of workmanship was
acceptable throughout.

a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building fuspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location oflot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c.;The work covered by this application may not be commenced before issuance of Building Permit.
d.-lJpon approval of this application, the Building fuspector will issue a Building Permit to the applicant. Such a permit
shall be leept on the premises available for inspection throughout the work.
'/:. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building fuspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Dear Mr. Dill:
The Southold Town Board of Trustees reviewed the survey prepared by John C. Ehlers
last dated February 10, 2005 and determined the proposed construction of a single-
family dwelling, in-ground swimming pool, and associated improvements to be out of the
Wetland jurisdiction under Chapter 97 of the Town Wetland Code and Chapter 37 of the
Town Code.
Therefore, in accordance with the current Tidal Wetlands Code (Chapter 97) and the
Coastal Erosion Hazard Area (Chapter 37) no permit is required. Please be advised,
however, that no construction, sedimentation, or disturbance of any kind may take place
seaward of the tidal wetlands jurisdictional boundary or seaward of the coastal erosion
hazard area as indicated above, without a permit. It is your responsibility to ensure that
all necessary precautions are taken to prevent any sedimentation or other alteration or
disturbance to the ground surface or vegetation within Tidal Wetlands jurisdiction and
Coastal Erosion Hazard Area, which may result from your project. Such precautions
may include maintaining adequate work area between the tidal wetland jurisdictional
boundary and the coastal erosion hazard area and your project or erecting a temporary
fence, barrier, or hay bale berm.
However, any activity within 100' of a Wetland line or seaward of the Coastal Erosion
Hazard Area would require permits from this office.
This determination is not a determination from any other agency.

Dear Mr. Dill:
Based on the information you have submitted, the New York State Department of Environmental
Conservation has made the following determination.
The portion of the referenced property landward of the topographic crest of the bank in excess of 10 feet
in elevation, shown as the top of slope on the survey prepared by John C. Ehlers last revised on 6/26/01,
is beyond the jurisdiction of Article 25 (Tidal Wetlands).
Therefore, in accordance with the current Tidal Wetlands Land Use Regulations (6 NYCRR Part 661) no
permit is required under the Tidal Wetlands Act. Please be advised, however, that no construction,
sedimentation, or disturbance of any kind may take place seaward of the tidal wetlands jurisdictional
boundary, as indicated above, without a permit. It is your responsibility to ensure that all necessary
precautions are taken to prevent any sedimentation or other alteration or disturbance to the ground

Dear Tony,
In completion of my contract with you for structural design services on your new home, I made
four visits to the site to observe ttle progress of construction and evaluate its conformanCe to the
structural plans' and specifications. Those plans and specifications were based upon the
following design codes and standards;
1. Residential Code of New York State
2. ASCE Standard Minimum Design Loads for Buildings aOO Other
Structures (98)
3. American Forest and Paper Association - Wood Frame Construction
Manual, High Wind Edition
4. American Institute of Steel Construction
My site visits were undertaken in November, 2005 (at the completion of the .steel frame),
January, 2006 (completion' of first floor framing), April, 2006 (completion of roof framing) and
November, 2006 (completion of all framing within the house). Within normal construction
tolerances, all construction conformed to the original plans; there were no field changes made.
Both the steel and wood framing was e.rected according to the plans and specifications. Steel
connections and reinforcements strictly followed the structural details. Hurricane clips, tie-
downs, strapping and nailing of the wood framing was in accordance with the referenced codes
and the project nailing schedule. In my professional opinion the quality 9f workmanship was
acceptable throughout.
