ft. of existing timber bulkhead with vinyl bulkhead; construct approx. 102 linear ft. of vinyl bulkhead
immediately landward of existing timber bulkheading to be cut to grade; and backfill with approx. 15 cy.
clean sand to be trucked in from an upland source. Located: 320 Sailor's Needle Rd" Mattituck.

The proposed action has been reviewed to Chapter 268, Waterfront Consistency Review of the Town of
Southold Town Code and the Local Waterfront Revitalization Program (L WRP) Policy Standards. Based
upon the information provided on the L WRP Consistency Assessment Form submitted to this department,
as well as the records available to me, it is my recommendation that the proposed action is
CONSISTENT with the Policy Standards and therefore is CONSISTENT with the LWRP.
Pursuant to Chapter 268, the Board shall consider this recommendation in preparing its written
determination regarding the consistency of the proposed action.

Attached please find an LWRP Consistency Assessment Form, which is being submitted in
association with an application to the Board of Trustees for a Town Wetlands Permit to replace
an existing timber bulkhead with a vinyl bulkhead.
I have indicated that all policies are "Not Applicable," because pursuant to Town Code ~95-5(A)
and the corresponding definitions of "action" and "minor action" by ~95-3, the submission of a
L WRP Consistency Assessment Form is not required for this application, as the proposed work
entails only "maintenance or repair involving no substantial changes in an existing structure or
facility,"

LWRP CONSISTENCY ASSESSMENT FORM
A. INSTRUCTIONS
I. All applicants for permits* including Town of South old agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supplement other information used by a Town of South old agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
2. Before answering the questions in Section C, the preparer of this form should review the exempt
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A orooosed action will be evaluated as to its significant
beneficial and adverse effects upon the coastal area (which includes all of Southold Town).
If any question in Section C on this form is answered "yes" or "no", then the proposed action will
affect the achievement of the L WRP policy standards and conditions contained in the consistency
review law. Thus. each answer must be explained in detail.listine both supportine and non-
supportine facts. If an action cannot be certified as consistent with the L WRP policy standards and
conditions, it shall not be undertaken.
A copy of the L WRP is available in the following places: online at the Town of Southold's website
(southoldtown.northfork.net), the Board ofTrustees Office, the Planning Department, all local
libraries and the Town Clerk's office.
