If you have any questions regarding the information to be submitted or the calculations required for this form, please contact the Regional Water Engineer at the address and phone number hsted on the RECORDING, REPORTING AND MONITORING page of your SPDES Permit. 1. Flow: Comoare the annual avera1!e flow to the flow limit in the oermit: All flow calculations must be made based on total flow. For WWTFs with one monthly average flow limit, calculate the averagc annual now by averaging the 12 monthly average flows reported on the monthly DMRs for calendar year 2007. For WWTFs monitoring quarterly average flow, calculate the average annual flow by averaging the 4 quarterly flows for 2007. For WVv'TFs monitoring semiannual flow, calculate the average aIUluaI flow by averaging the 2 semiannual measurements for 2007. The dates used for the seasonal flow rate calculations should be included on the form in the space provided. If either of the seasonal calculations exceed 95% of the design flow, the "Yes" box must be checked and a Flow Management Plan must be prepared in accordance with 6 NYCRR Part 750-2.9(c)(I) and submttted to the Regtonal Water Engineer no later than August 1, 200B. If a Flow Management Plan has been previously submitted to the Department, indicate the date of submission on the line in Item l.d. The design flow of the treatment facility shall be in accordance with that listed in the most recent design documents or treatment plant ratmg submitted to and approved by this Department. If the design flow for the treatment facility is unknown, the design flow should be estimated using the "Recommended Standards for Wastewater Facilities, 2004 Edition" (Ten States Standards), which can be ordered from this website: htto://\vww.hes.org . Loadings: Compare the design BOD and TSS influent loadin2s to the actual BOD and TSS loadin2s: The design influent loadings are not included on the preprinted DMR. These numbers may be found in the facility's engineering design documents, O&M plans, or in more recent engineering reports if the treatment facility has had a recent upgrade. Use the most recent design loadings data available for purposes of this calculation. If you cannot locate the facility's design influent loadings enleria, please conlaclthe plant engineer. If the design loadings for the treatment facility are unknown, the loadings should be estimated using the "Recommended Standards for Wastewater Facilities, 2004 Edition" (Ten States Standards), which can be ordered from this website: htto://www.hes.orl! . If the wastewater treatment facility uses Biochemical Oxygen Demand (BOD) as a measure of treatment system performance, use BOD loudings in these calculations. If the facility uses CBOD as a measure of treatment system performance, use CBOD loadings in these calculations. Facilities with sand filters systems or other systems with no DMR requirements should check the box in Item 2. To calculate the actual influent loadings in lbs/day, facilities that perform daily sampling should multiply the average monthly concentration on the DMR (10 mg/I) by the average monthly flow (in MGD) and a conversion factor of8.34. For facilitIes With less than daily sampling, i.e., monthly, weekly, etc., the daily flow from the POTW on the date that the sample was collected shall be used to determine the loadings. These individual daily loadings should be averaged for the monthly actual influent loading. For WWTFs monitoring quarterly, semiannual, or annual loadings, calculate the actual loadings by averaging the loadings as reported during calendar year 2007. I f either of the seasonal calculations exceed the design loading, the "Yes" box must be checked. Should the actual influent loading for either BOD (or CBOD) or TSS exceed the design influent loading, you may include an explanation for these exceedances. Check the "Yes" box in Item 4.b. if a written explanation is attached. If the facility has previously submitted a Plan for Future Growth. indlcate the date that the plan was submitted in Item 3.d. The requirements for the plan for future growth contamed in 6 NYCRH. Part 750-2.9(c)(2) are attached for reference. 3. Implementation of Plan for Future Growth and Sewer Moratorium: Review the facility's DMRs 10 determine whether the discharge from the 'POTW has exceeded a SPDES permit effluent limit for BOD, CBOD, Ultimate Oxygen Demand (including 28 day BOD), or Total Suspended Solids for any four or more months during two consecutive calendar quarters during 2007. If the answer to this question is "Yes", an immediate moratorium on additional sewer connections shall be imposed and the approval of sewer connections to the POTW must cease immediately. The requirements for the sewer connection moratorium contained in 6 NYCRR Part 750-2.9(c)(3) are attached for reference. 4. Additional Information: If the facility is currently under an Order on Consent with the Department that addresses flow and/or loadings concerns, check the "Yes" box in Item 4.8. This plan shall be certified by a professional engineer licensed to practice in the State of Nev-,' York and endorsed by the chief fiscal officer of the municipality. The provisions of the plan may reflect new efforts or may refer to existing, ongoll1g efforts, The flow management plan shall, at a minimum, include provisions for: (a) A statement to the effect that the permittee has the authority in all parts of the POTW service area to implement or cause to be implemented the provisions of this subdivision or, if the permittee does not have such authority, a proposed schedule, not to exceed three years, to obtain such authority or a statement from the permittee's designated legal representative that existing law precludes the permittee from obtaining such authority; (b) An inventory of all known facilities/projects that have applied to connect to the sewer system and a determination if there is capacity for connection; (c) A schedule of implementation for all flow reduction measures identified herein; (d) A map delineating the service area as defined; and (e) A description of information that will be reported during implementation of the plan to the Regional Water Engineer and a schedule for such reporting. (ii) The l1C1\V management plan required by subparagraph (i) of this paragraph shall also include provisions for implementation of any or all of the following that are necessary to stabilize influent flows below design flows: (a) Water conservation measures to reduce customer usage by measures including but not limited to customer metering, meter calibration, retrofitting existing plumbing fixtures with water conservation fixtures and revision of water rate structures; (b) Reduction of infiltration and inflow through continuous measures including but not limited to sewer system metering, evaluation and rehabilitation, removal of roofleaders and footing drains from separate sanitary sewers and installation of separate storm sewers; (c) Prevention of future sources of infiltration and inflow where feasible through measures including but not limited to implementation of standards for sewer installation and requirements to provide for adequate drainage from roofleaders and footing drains in new construction; (d) Measures to maximize sewer system and sewage treatment works capacity at a minimum cost; and/or (e) Approvable engineering reports and/or plans and specifications to assure annual average flows do not exceed 95 percent of the POTW treatment plant design flow. (I) Capital improvements necessary to assure annual average flows do not exceed 95 percent of the POT\V treatment plant design now. (iii) W ithm 90 days of submittal to the Regional Water Engineer of the plan required under subparagraphs (i) and (ii) of this paragraph. the peffi1ittee shall begin to implement the provisions of said program in accordance with the proposed schedule or cause the provisions of said program to be implemented by another party. (iv) The RegIOnal \Valer Engineer may object to the plan, or implementation of the plan, submitted in accordance with subparagraph (i) and (ii) of this paragraph if the plan does not provide for substantive and effective measures to reduce hydraulic loading to the POT\V. \\/11h111 90 days of receIpt of written notification from the Regional Water Engineer documenting the aspects of the plan that must be revIsed, the pert11ltlee shall submit a revised plan that addresses the department's objection(s). Part 750 -2.9 (c) POTW DESIGN, PLANNING AND FLOW MANAGEMENT (2) Planning (i) Within 120 days of when the permittee determines that the actual influent mass loading of Biochemical Oxygen Demand or Total Suspended Solids to a POTW has reached or exceeded the design influent loading for those parameters for any eight calendar months dunng a calendar year, the permittee shall submit a plan for future growth at the POTW. The plan shall include: (a) Provisions for obtaining any necessary funding; and Part 750 - 2, Page 61 Effective May 11, 2003 (b) Provisions for preparation and submission to the Regional Water Engineer of approvable engineering reports and/or plans and specifications to provide for growth of discharges in the POTW service area. (c) A demonstration of the permittee"s ability to impose a connection moratorium in any and all parts of the service area or, if the penllittee does not have such authority, a proposed schedule, not to exceed three years, to obtain such authority or a statement from the permittee's designated legal representative that existing law precludes the permittee from obtaining such authority. (ii) The Regional Water Engineer may object to the plan, nr implementation of the plan, submitted in accordance with subparagraph (i) of this paragraph if the plan does not provide for substantive and effective measures to accommodate future grov.rth of discharges from the POTW service area. Within 90 days of receipt of written notification from the Regional Water Engineer documenting the aspects of the plan that must he revised, the permittee shall submit an approvable, revised plan that addresses the department's objection(s). (iii) Within 90 days of submittal to the Regional Water Engineer of the plan required under subparagraph (i) ofthis paragraph, the pen11lttee shall begin to implement the plan to obtain the authority required under clause '(c)' of subparagraph (i) of this paragraph. (3) Plan Implementation and Sewer Connection Moratorium For POTWs that have exceeded the design influent loading criteria set forth in paragraph (2) of this subdivision, within 90 days of when the permittee determines that, in accordance with the annual review required by paragraph (4) of this subdivision, that the effluent discharge from a publicly owned treatment works has exceeded a SPDES permit limit for Biochemical Oxygen Demand or Ultimate Oxygen Demand for any four or more months during two consecutive calendar quarters, or a SPDES permit limit for Total Suspended Solids for any four or more months during two consecutive calendar quarters, the permittee shall: (i) Begin to implement the plan developed in accordance with paragraph (2) of this subdivision or in accordance with subparagraph (i) of that paragraph; and (ii) Cease the further approval of sewer connections to the POTW.
