This application must be filled in by typewriter or ink and submitted (0 the Building Department with the following:
A. }Ior new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 fonn).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 211 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or bnildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

I have attached the revised deck stair plans and the letter from the architect
concerning the insulation and venting of the ceiling in the back master bedroom for the
project at 620 North Drive, Mattituck ( Permit # 32691 Z). From our last discussion of
your requirements this brings us up to date with everything you said you needed to get us
to the final inspection. I will notify you when we are ready for the final inspection.

Dear Ms. Moore:
The Board of Town Trustees took the following action during its regular meeting held on
Wednesday, December 13, 2006 regarding the above matter:
WHEREAS, Patricia C. Moore, Esq. as agent for NORMAN AND ADELE GRUDMAN
applied to the South old Town Trustees for a permit under the provisions of Chapter 275
of the South old Town Code, the Wetland Ordinance of the Town of Southold,
application dated October 24, 2006, and,
WHEREAS, said application was referred to the Southold Town Conservation Advisory
Council and to the Local Waterfront Revitalization Program Coordinator for their findings
and recommendations, and,
WHEREAS, the Board of Trustees has furthered Policy 6 of the Local Waterfront
Revitalization Program to the greatest extent possible through the imposition of the
following Best Management Practice requirements: the installation of drywells to contain
the roof run-off and the installation of a hay bale line during construction, and,
WHEREAS, a Public Hearing was held by the Town Trustees with respect to said
application on December 13, 2006 at which time all interested persons were given an
opportunity to be heard, and,
WHEREAS, the Board members have personally viewed and are familiar with the
premises in question and the surrounding area, and,

WHEREAS, the Board has considered all the testimony and documentation submitted
concerning this application, and,
WHEREAS, the structure complies with the standards set forth in Chapter 275 of the
Southold Town Code,
NOW THEREFORE BE IT,
RESOLVED, that for the mitigating factors and based upon the Best Management
Practice requirements imposed above, the Board of Trustees deems the action to be
Consistent with the Local Waterfront Revitalization Program pursuant to Chapter 268-5
of the Southold Town Code, and,
RESOLVED, that the Board of Trustees approve the application of NORMAN AND
ADELE GRUDMAN to construct an addition over the existing dwelling, a new wood
deck with steps to grade, a new garage addition, and (2) one-story additions to the
dwelling, with the condition of drywells to contain run off, hay bales during construction,
and all as depicted on the plans prepared by Gao Enterprises, Inc. and last revised
10/23/06.
Permit to construct and complete project will expire two years from the date the permit
is signed.
Fees must be paid, if applicable, and permit issued within six months of the date of this
notification.
Inspections are required at a fee of $50.00 per inspection. (See attached schedule.)
This is not a determination from any other agency.

TERMS AND CONDITIONS
The Permittee, Norman and Adele Grudman, 620 North Drive, Mattituck, as part of the
consideration for the issuance ofthe Permit does understand and prescribe to the following:
I. That the said Board of Trustees and the Town of Southold are released from any and all
damages, or claims for damages, of suits arising directly or indirectly as a result of any
operation performed pursuant to this permit, and the said Permittee will, at his or her own
expense, defend any and all such suits initiated by third parties, and the said Permittee
assumes full liability with respect thereto, to the complete exclusion of the Board of
Trustees of the Town of Southold.
2. That this Permit is valid for a period of24 months, which is considered to be the estimated
time required to complete the work involved, but should circumstances warrant, request for
an extension may be made to the Board.
3. That this Permit should be retained indefinitely, or as long as the said Permittee wishes to
maintain the structure or project involved, to provide evidence to anyone concerned that
authorization was originally obtained.
4. That the work involved will be subject to the inspection and approval of the Board or its
agents, and non-compliance with the provisions of the originating application may be cause
for revocation of this Permit by resolution of the said Board.
5. That there will be no unreasonable interference with navigation as a result of the work
herein authorized.
6. That there shall be no interference with the right of the public to pass and repass along the
beach between high and low water marks.
7. That iffuture operations of the Town of South old require the removal and/or alterations in
the location of the work herein authorized, or if, in the opinion of the Board of Trustees,
the work shall cause unreasonable obstruction to free navigation, the said Permittee will be
required, upon due notice, to remove or alter this work project herein stated without
expenses to the Town of Southold.
8. The Permittee is required to provide evidence that a copy of this Trustee permit has been
recorded with the Suffolk County Department of Real Properties Office as a notice
covenant and deed restriction to the deed of the subject parcel. Such evidence shall be
provided within ninety (90) calendar days of issuance of this permit.
9. That the said Board will be notified by the Permittee of the completion of the work
authorized.
10. That the Permittee will obtain all other permits and consents that may be required
supplemental to this permit, which may be subject to revoke upon failure to obtain same.

Based on the infonnation you have submitted, the Department of Environmental COllSeI\lation has
detennined that the portion of the referenced property located landWlUd of the crest of the shoreline bank
which exceeds ten feet above mean sea level, as shown on the January 19,2006 survey by John Ehlers,
L.S., last revised July 6, 2006 is beyond Tidal Wetlands Act (Article 25) jurisdiction. Therefore, in
accordance with tbe current Tidal Wetlands Land Use Regulations (6 NYCRR Part 661), no pennit is
required for construction, excavatiOn, grading or .other site development activities located landward of
the jurisdictional boundary: . . .

d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
