This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new nse:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dep!. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certirying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land nses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building h1spector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location oflot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit,
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Buil~g Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Building Zone Ordinance ofthe Town of South old, Suffolk County, New York, and other applicable Laws, Ordinances or

I have reviewed your department's oomments with respect to our construction
project and have added the following information:
1. Separation of large storage area from main area
We will provide a two hour wall fire separation between this large storage area
and the maln area of Daniel Gale's space.
The rating between Daniel Gale's space and the mercantile M next door shall
remain consistent with our documents of August 30, 2007.
2. Interior finishes
The interior finishes for Group B In an unsprlnklered structure range from A-flnlsh
to C-flnish. The finishes we propose for our space will meet the N"ew York
State Building Code.

As per your recent inquiries with respect to our application before your
department, I have put together the information you require.
1. Building by Construction C'......ificatlon
From our investigation it appears the building falls Into two construction
categories:
The lower floor is constructed of brick and block exterior walls with wood roof
rafters. This section of the building is Type III B.
The upper floor is constructed of frame construction making it a Type V. At one
time the original house that stood there (Type V) was surrounded by the Type
III construction.
2. Buildina by Occuoancv CIA""lfIcatlon
The space that Daniel Gale Realty will be taking will be classified as business
B.
The adjoining space next door is classified as Mercantile M.
The second floor, which projects over part of our space, is Residential R-2 (two
apartments).
According to Table 302.3.3 there should be a two hour fire rating between the
various occupancies.

3. Fire Ratlna lceiling/roof construcllonl
a) Rating betWeen first floor B and second floor R-2:
The best rating between these two areas that I can achieve is a one hour.
According to Table 719.(3) I would achieve this by Installing two layers of SJ8"
Type X gypsum on the ceiling under the apartments.
b) Rating betWeen Business B and Mercantile M:
The best rating between these two areas I could achieve would be one hour. I
could put two layers of 5J8" Type X gypsum board between the tenant
spaces to achieve the full hour. I w01.ild need to Install two layers on both sides
which Is not possible as the space Is currently occupied. Pernaps we could
install out layers new and when the space next door becomes available the
new tenant would install gypsum board on that side.
(j) Separation of Star. Areas from Main Area
According to Table 302.3.3 occupancy separation need not be provided for
incidental storage, areas within group B If area is less than 1000 feel
The large storage area indicated on the plan is 501 square feet.
5. Emervencv / Exit UQhts
Our design does indicate emergency/exit lights. We Wllllnstall this on a
dedicated circuit as directed.
Alarm System
We are instalHng a central alarm system for this space. The system can
incorporate smoke, fire and carbon monoxide presence.
I trust that these items address your department's questions. While we are
unable to bring total conformity to the codes, I must point out that the
anticipated use of the space is much safer than the restaurantlkltchen conditions
that previously existed.
