A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept, of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters,
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of I% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied;the Building Inspector shall state the reasons tlferefor in writing to the applicant.

1 have reviewed the file and inspected the site referenced above for drainage and surface water runoff
concerns. This property is located in a Flood Zone"X"which is not regulated,but it is surrounded by a Zone of
AE(Elev.8)that is. The roads within this subdivision were dedicated to the Town approximately 10 years ago.
At that time,all road&site drainage was directed through a low lying ditch systems that diverted all run-off to the
low vegetated wetland areas surrounding the Subdivision.
At that time, the lot referenced above had a ditch system traversing the site. This ditch allowed the
roads and other surrounding lots to adequately drain off. Since the excavations for construction began, the
drainage system has failed. Ditches were filled in and water has backed up creating flooding conditions that had
never existed prior to excavations and grading of this site. In addition,the area excavated for the basement or
crawl space has been totally filled in with water. Due to the amount of clayed material disturbed during the
excavation and the amount of water present,any foundations placed or constructed to support new construction
at this site will need to have project specific engineering performed by a licensed professional to verify the
stability of the foundation system proposed for new construction. Simply filling the hole with excavated clayed
material would not be considered suitable or stable enough to support any new construction.
With the amount of clay in the area, it is highly unlikely that standard drainage systems would provide
adequate recharge to solve the current drainage problems. I would recommend that the applicant be required
to provide a complete drainage design prepared by a licensed engineer that would rectify the above referenced
site drainage problems.

1 have reviewed the file and inspected the site referenced above for drainage and surface water runoff
concerns. This property is located in a Flood Zone"X"which is not regulated, but it is surrounded by a Zone of
AE(Elev.8)that is. The roads within this subdivision were dedicated to the Town approximately 10 years ago.
At that time,all road&site drainage was directed through a low lying ditch systems that diverted all run-off to the
low vegetated wetland areas surrounding the Subdivision.
At that time, the lot referenced above had a ditch system traversing the site. This ditch allowed the
roads and other surrounding lots to adequately drain off. Since the excavations for construction began, the
drainage system has failed. Ditches were filled in and water has backed up creating flooding conditions that had
never existed prior to excavations and grading of this site. In addition,the area excavated for the basement or
crawl space has been totally filled in with water. Due to the amount of clayed material disturbed during the
excavation and the amount of water present,any foundations placed or constructed to support new construction
at this site will need to have project specific engineering performed by a licensed professional to verify the
stability of the foundation system proposed for new construction. Simply filling the hole with excavated clayed
material would not be considered suitable or stable enough to support any new construction.
With the amount of clay in the area, it is highly unlikely that standard drainage systems would provide
adequate recharge to solve the current drainage problems. I would recommend that the applicant be required
to provide a complete drainage design prepared by a licensed engineer that would rectify the above referenced
site drainage problems.

RE: New Residence Under Construction: Sec 57,Block 01,Lot 21
Dear Mr.Verity:
Thank you for taking the time to meet with me last Friday regarding the above referenced project.As I
mentioned I own the property at 415 Blue Marlin Drive(lot#23)which is directly to the south of the house
and detached garage under construction.
I have enclosed a photograph taken from my back yard which shows the excessive height of the foundation
for the garage.The top of foundation on the south side is between 4 to 6 feet above grade. I request that the
Building Department carefully review this condition to confirm that construction is within the height
restriction of the code.
In addition,I am the concerned about stormwater runoff that will flow onto my property from this project.
Apparently the Owner is resolving his own water problems by raising grade around the entire property thus
causing water runoff to the neighboring properties. I suspect the Town code requires properties to maintain
their own water and I request this issue be investigated as well.
I called the Owner of the subject parcel to discuss these issues in a friendly manner.I left a message on
their voice mail but they never returned the call.
If I can provide any additional information to help in the review of this request please let me know.
