This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifYing that the solder used in system contains less than 2/10 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy,
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building ,Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required,

Dear Sir/Madam:
In accordance with the provisions of Section 10 of the
Rivers and Harbors Act of 1899 (33 USC 403), Peconic Land Trust
was issued Department of the Army Permit Number 2004-00850 by the
District Engineer on November 2, 2004. This permit authorized
reface approximately 280 feet of timber bulkhead, within an
existing hatchery trough, with PVC corrugated sheathing. Replace,
in-kind and in-place, three (3) weirs each approximately 20 feet
in length. The front weir is constructed in a V-shape. Replace,
in-kind and in-place, a conduit grate. Remove and replace,
in-kind and in-place, the quonset hut building. Maintenance
dredge approximately 140 cubic yards of material from a 700
square foot area, between the weirs and the quonset hut, to a
depth of 6 feet below the plane of mean low water. Dredge 265
cubic yards of material from a 2,400 square foot area within the
hatchery trough to a depth of 4 feet below the plane of mean low
water. Excavate approximately 80 cubic yards of material from a
40 foot long culvert connecting the hatchery trough and the
adjacent lagoon south of the hatchery. All of the resultant
dredge material will be disposed of on-site at an upland area.
The work site is located in an unnamed lagoon off of Shelter
Island Sound, Town of Southold, Suffolk County, New York.
The New York District has reviewed your September 1, 2006
request for an extension of the original permit and agrees that
such an extension would not be contrary to the general public
interest. Accordingly, the subject permit is hereby specifically
modified to extend the expiration date to two (2) years from the
date of this letter, and the modification of this permit shall be
known as Department of the Army Permit Number 2004-00850-1. All
other permit conditions to which the authorized work was made
subject shall remain in effect.
This letter shall be added to all copies of the permit, including
those at the work site. If any questions should arise concerning
this matter, please contact Russell C. Smith, of my staff, at

Remove and replace in-place 3 existing weirs/wave breaks. Remove and replace in-place 280 linear feet of
existing functional bulkhead within a hatchery trough. Dredge to 6' below mean low water within the weir
area. Dredge to 4' below mean low water within the hatchery trough and existing conduit with resultant 580
cu. yds. of dredged material to be placed at approved on-site location. Remove and replace existing
Quonset hut building with 12' high 22' x 44' steel building. All work must be done in accordance with the
attached plans prepared by Coastal Consultants last revised November 30, 2005 and stamped approved by
NYSDEC on January 18, 2006.
Your proposed project, as shown on the plans prepared by Coastal Consultants last revised
November 30, 2005, is more than 100 feet from regulated freshwater wetlands.
By acceptance of this permit, the permittee agrees that the permit is contingent upon strict compliance with the ECL, all
applicable regulations, the General Conditions specified (see page 2 & 3) and any Special Conditions included as part of
this permit.

The following action was taken by the Southold Town Board of Trustees at their Regular
Meeting held on Wed, May 16, 2007:
RESOLVED, that the Southold Town Board of Trustees APPROVE the Amendment to
Permit #6014 to dredge a 4' wide X 3' deep X 150' long channel in lagoon south of the
hatchery building, and in accordance with the plans prepared by Gregg Rivara dated

Dear Sir/Madam:
In accordance with the provisions of Section 10 of the
Rivers and Harbors Act of 1899 (33 USC 403), Peconic Land Trust
was issued Department of the Army Permit Number 2004-00850 by the
District Engineer on November 2, 2004. This permit authorized
reface approximately 280 feet of timber bulkhead, within an
existing hatchery trough, with PVC corrugated sheathing. Replace,
in-kind and in-place, three (3) weirs each approximately 20 feet
in length. The front weir is constructed in a V-shape. Replace,
in-kind and in-place, a conduit grate. Remove and replace,
in-kind and in-place, the quonset hut building. Maintenance
dredge approximately 140 cubic yards of material from a 700
square foot area, between the weirs and the quonset hut, to a
depth of 6 feet below the plane of mean low water. Dredge 265
cubic yards of material from a 2,400 square foot area within the
hatchery trough to a depth of 4 feet below the plane of mean low
water. Excavate approximately 80 cubic yards of material from a
40 foot long culvert connecting the hatchery trough and the
adjacent lagoon south of the hatchery. All of the resultant
dredge material will be disposed of on-site at an upland area.
The work site is located in an unnamed lagoon off of Shelter
Island Sound, Town of Southold, Suffolk County, New York.
The New York District has reviewed your September 1, 2006
request for an extension of the original permit and agrees that
such an extension would not be contrary to the general public
interest. Accordingly, the subject permit is hereby specifically
modified to extend the expiration date to two (2) years from the
date of this letter, and the modification of this permit shall be
known as Department of the Army Permit Number 2004-00850-1. All
other permit conditions to which the authorized work was made
subject shall remain in effect.
This letter shall be added to all copies of the permit, including
those at the work site. If any questions should arise concerning
this matter, please contact Russell C. Smith, of my staff, at
