This application must be filled in by typewriter or ink and submitted to the Building Depariment with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dep!. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of I % lead.
S. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Mr. Mohr:
The Planning Board, at their work session on Monday October 22, 2007, discussed the above
refere~ced site p~. As}'Ju are aware, the Planning Bo~ has. reviewed various as-built site plans
and discussed vanous staff reports at numerous work sessIons m an effort to issue final site plan
satisfaction. Most recently, the Planning Board is in receipt of the enclosed letter from Joseph
Fischetti, P.E. dated October 26,2007 which the Planning Board reviewed at the work session on
Monday, October 29, 2007.
This is to inform}'Ju that the site plan requirements have been satisfied and include the following
iterm:
. The Planning Board accepts the October 26, 2007 letter from Joseph Fischetti, P.E. certifying
the installation of all necessary drainage for the existing 40.2' x 52.1' 1-story metal frarne
building located at the rear of the property.
. The Planning Board does not acknowledge or approve any encroachment, permanent or
temporary, onto neighboring properties or the Suffolk County right of way.
. Dumpsters shall not block any approved parking spaces, building entrances, etc.
. All existing ponds and waterfalls must meet Town Code and NYS Building Code and shall be
subject to approval of the Southold Town Building Inspector. If necessary, the applicant
agrees to meet all requirements of the Building Department and obtain all necessary
approvals/ pennits.
. The existing 10' gazebo must meet Town Code and NYS Building Code and shall be subject
to approval of the Southold Town Building Inspector. If necessary, the applicant agrees to
meet all requirements of the Building Department and obtain all necessaryapprovals/pennits.
. All handicap parking stalls, access aisles and signage must comply with New York State Code
and ADA requirements.
. Outdoor lighting shall be shielded so that the light source is not visible from adjacent
properties and roadways. Ughting fJXtures shall focus and direct the light in such a manner as
to contain the light and glare within property boundaries. All1ighting must meet town code
requirements.

. As per Town Code $280-127, "any change in use or intensity of use which will affect the
characteristics of the site in terms of parlring, loading, access, drainage, open space or utilities
will require site plan approval".
· The applicant agrees to obtain all necessary sign pennits from the Building Department. All
signs shall meet Southold Town Zoning Codes and shall be subject to approval of the
Southold Town Building Inspector.
. No change in landscaping is permitted as part of this satisfaction letter. Any future change in
landscaping must be approved by the Planning Bow following submission of a written
request by the applicant.
. As previously discussed and agreed upon, the existing landscaping must mature to minimize
the impact of the existing rock wall along County Road 48. The applicant agrees the rock
wall will be substantially covered with foliage within two (2) years of this approval. The
Planning Bow reserves the right to require additional landscaping if the rock wall is not
substantially covered within two (2) years.
. As per landscape survivability, the applicant agrees to replace any of the landscaping which
dies within three (3) years of planting.
Please be aware all existing construction must comply with Town Code prior to issuance of a
Certificate of Ckcupancy. This letter does not condone any changes from the as-built site plan and
approvals from other agencies if required.

INSTRUCTIONS
. s application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location oflot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in wrlting, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of f{ Building Permit pursuant to the
Building Zone Ordinance of the Town of Southold, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The
applicant agrees to comply with all applicable laws, ordinances, building code, housing code, and regulations, and to admit
authorized inspectors on premises and in building for necessary inspections.
