APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage -disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9,1957) non -conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and a consent to inspect signed by the applicant. If a Certificate of Occupancy
is denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Mr. Fish,
The existing home at 1110 Orchard Street has been constructed in four (4) sections at
different times. The original section is the main two story structure built in 1830, a side
addition, 13' x 12' was constructed in 1890, added to that was an 18' x 9' structure built prior to
1957. The newest addition was built with a permit noted as 1987.
The 1830 structure has a full basement under ''/z of its structure and a crawl that is not
fully visible. From the basement I can see into part of the crawl space of the 1890 and pre
1957 sections and a small northerly portion of the 1830 crawl. There is visible damage to one
floor joist in the 1890 section just inside the crawl on the left side that needs repair at this time,
but is not unsafe. The remaining visible floor structures seem to be in fair condition and do not
have visible damage.
I walked the first floor of the dwelling and while there are areas of the floor that have
moved over the years and are out of level, this is not remarkable for a home of this age. To the
best of my ability I found no areas of the floor structure that were unsafe at this time. While the
current owner has provided maintenance, all homes of this age require constant repairs to
keep up with the aging process to keep the homes safe.
