Dear Ms. Finnegan:
Thank you for contacting Nelson, Pope & Voorhis for environmental consulting services regarding SEQRA
compliance for the requested water main connection at the above referenced site. As you know, NP&V
completed an environmental assessment on behalf of the Town for the adoption of the 2006 Water Supply Plan
Map and we are familiar with the issues related to future eonnections to transmission/distribution lines. The 2006
Water Map indicated transmission mains in seven locations not shown on the 2000 Water Map including the
extension that is adjacent to the subject property. The analysis found that the subject transmission line and
associated distribution lines would not cause an undue increase in potential development, would serve areas
consistent with Town policy, and would ensure protection of public health; as a result, this aetion was not
expected to result in a significant adverse environmental impact.
The proposed action under SEQRA is expected to be classified as an Unlisted Aetion and requires the preparation
of an Environmental Assessment Form to identify potential impacts. It is expected that the SEQRA review related
to the eonnection for the new Cutchogue Fire District Station will draw on the existing analysis contained in the
assessment. It is noted that this is a unique application, since the application is to the Town Board for connection
to the water distribution system and, to our knowledge, no site plan application is pending. Thus, the Town
Board's decision will be limited at this time to this single action, as will the SEQRA review. In this case
therefore, it will be necessary for a separate SEQRA review to occur at the time of site plan application, since we
are unable to analyze the potential impacts of the development until such an application is submitted.
Our scope of services for this SEQRA review includes the following tasks:
1. Review proposal materials submitted by the Cutchogue Fire District and Architect - Planner; contact
applicant's architect if additional information regarding expected water usage is needed. Review 2006
Map and Environmental Assessment prepared in June 2lX!? regarding the extension refereneed above.
2. Prepare Environmental Assessment Form, Parts I, II and Part III.
3. Prepare Determination of Signifieance for consideration by the Town Board.
The fee for our services is $1,000.00. As discussed, this fee assumes that we will prepare a concise Part III
document to expand upon the analysis included in the review of the 2006 Water Map. If this is satisfaetory, please
provide appropriate authorization and we will commence review immediately. It is expected that the review will
be complete within 2 weeks of authorization. Thank you for the opportunity to assist on this project. Please feel

of Fire Commissioners ofthe Cutchogue Fire District pursuant to SEQR. Also enclosed are
copies of the proposed site and building plans, and Part I of the Full Environmental Assessment
Form, dated October 8, 2007, prepared by the Fire District's architect, Martin F. Sendlewski.
I have been directed by the Board of Fire Commissioners to forward the enclosed items to
you as part of a coordinated review under SEQR and to request any comments you have
concerning this project within 30 days. The Board of Fire Commissioners intends to establish
itself as lead agency for this project.

At a Regular Meeting of the Board of Fire Commissioners of
the Cutchogue Fire District, Town of Southold, Suffolk County,
New York, held at the Cutchogue Fire House, 260 New Suffolk Road,
Cutchogue, New York on the 8th day of October, 2007, at 8:00
P.M., prevailing time.

Also present were Matthew J. Martin, Secretary for the
Cutchogue Fire District, Peter Zwerlein, Treasurer for the
Cutchogue Fire District, Richard F. Lark, Esq., of Lark & Folts,
attorneys for the Cutchogue Fire District, Martin F. Sendlewski,
Architect-Planner.
The Chairman of the Board of Fire Commissioners, Sanford H.
Friemann, reviewed for the Board and those present that due to
the increased population in the Cutchogue Fire District, the Fire
Department had outgrown the facilities at New Suffolk Road, and
there was not sufficient space to add additional facilities at
this location, plus the need to provide more efficient and
effective response to alarms, approximately two years ago the
Fire Commissioners began an extensive search in cooperation with
the Fire Chiefs to locate an appropriate parcel of property to
fulfill the present and future needs of the Fire District.
Finally, after an intensive search, a 6_3 acre parcel was located
at 5455 Cox Lane, Cutchogue, New York, and as a result of
extensive negotiations the Fire District purchased same on
October 13, 2006_
The Chairman then reported that subsequently, Martin F.
Sendlewski, AlA, a specialist in Fire District Planning and
Design, was retained by the Board of Fire Commissioners to design
a site plan and firehouse facility at this location to fulfill
the needs of the Fire District. As a result, during the past
year the Fire Chiefs and Board of Fire Commissioners have met
numerous times with Martin F. Sendlewski and his staff to develop
a cohesive plan for a new firehouse facility at this location to
serve the public and, in particular, the residents of the
Cutchogue Fire District. As a result of all of these efforts
Martin F. Sendlewski has presented to the Board of Fire
Commissioners a site plan and floor plans dated October 8, 2007,
along with a Full Environmental Assessment Form dated October 8,
2007, for the consideration and approval of the Board of Fire
Commissioners. The Fire Commissioners, along with the Fire
Chiefs, have thoroughly reviewed these materials.
NOW, on motion of Commissioner Brewer, seconded by
Commissioner Zuhoski, it is
RESOLVED, that after reviewing the State Environmental
Quality Review Act (SEQR) and the site and building plans for the
new firehouse facility to be located at the Fire District
property at 5455 Cox Lane, Cutchogue, New York (SCTM District
2
1000, Section 84, Block 4, Lot 1), along with the Full Environ-
mental Assessment Form (EAF) Appendix A, all prepared and
submitted by Martin F. Sendlewski, AlA Architect-Planner, it is
determined:
1. The project for the New Firehouse Facility is
subject to SEQR, and the Board of Fire
Commissioners assumes lead agency;
2. The project does not involve a federal agency;
3. The project may involve one or more other agencies
as set forth in response to question 25 of the
Full Environmental Assessment Form attached
hereto;
4. The New Firehouse Facility project pursuant to
SEQR criteria is an Unlisted Action; however, it
is determined that coordinated review of the
project with other agencies is necessary;
and it is further
RESOLVED, that the Secretary of the Cutchogue Fire District
shall file with the Fire District's records a copy of this
resolution, as well as copies of the site and building plans
dated October 8, 2007, and Full Environmental Assessment Form
dated October 8, 2007, prepared by Martin F. Sendlewski, AlA; and
it is further
RESOLVED, that the Secretary of the Cutchogue Fire District
shall forward a copy of this determination of the Board of Fire
Commissioners, along with copies of the site and building plans,

and Full Environmental Assessment Form, to the agencies listed in
question 25 of the Full Environmental Assessment Form notifying
them that a lead agency must be agreed upon within 30 calendar
days, and further requesting that they comment on the project and
respond to the Board of Fire Commissioners within 30 days of
receipt of same.
The question of the adoption of the foregoing resolutions
Has put to a vote on roll call, which resulted as follows:

State Environmental Quality Review
FULL l:NVIRONMENT AL ASSESSMENT FORM
Purpose: The full EAF is designed to help applicants and agencies determine, in an orderly manner, whether a project or action may
be significant. The question of whether an action may be significant is not always easy to answer. Frequently, there are aspects of
a project that are subjective or unmeasurable. It is also understood that those who determine significance may have little or no formal
knowledge of the environment or may not be technically expert in environmental analysis. In addition, many wht;) have knowledge
in one particular area may not be aware of the broader concerns affecting the question of significance.
The full EAF is intended to provide a method whereby applicants and agencies can be assured that tile determination process
has been orderly, comprehensive in nature, yet flexible enough to allow introduction of information to fit a projoct or action.
Full EAF Components: The full EAF is comprised of three parts:
Part 1: Provides objective data and information about a given project and its site. By identifying basic project data, it assists
a reviewer in the anaiysis that takes piace in Parts 2 and 3.
Pan 2: Focuses on identifying the range of possible Impacts that may occur from a project or action. it provides guidance
as to whether an impact is likeiy to be considered small to moderate or whether it is a potentially-large impact. The
form also identifies whether an impact can be mitigated or reduced.
Pan 3: If any Impact in part 2 is identified as potentially-large. then Part 3 is used to evaluate whether or not the impact is
actually important.

Forwarded herewith are copies of the logs of two borings drilled at the above referenced
site.
The purpose of the subsurface investigation was to determine the nature and extent ofthe
underlying soil deposits and determine the structural engineering characteristics of the
soil at the site. A total of two (2) test borings were drilled to a depth of 27 feet each. A
2" diameter, 2'0" long split spoon sampler was advanced into the subsurface by the use
of an automatic 140 lb. hammer with a 30" drop. From the drops of the hammer, blow
counts required to advance the split spoon sampler over each 6" interval were recorded
and are shown on the boring logs. Continuous split spoon samples were taken for the top
6 feet and then every 5 feet thereafter to the final depth.
Our investigation revealed that the areas drilled are blanketed by from 18 inches to 30
inches of soft loam, underlain, generally, by a moderately dense naturally bedded sand
fonnation with traces of gravel extending to the deepest depth drilled.
No water, either natural or perched, was encountered in the boreholes at tile time the
work was done.
All soils below 3 feet will satisfactorily support foundation loads of 2 tons per square
foot.
Soil samples recovered during drilling operations will be stored in our lab for a period of
30 days after which they will be destroyed. During this period we will deliver these
samples to any prescribed location upon request.
