This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage -disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters,
4. Swom statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non -conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is

Dear Mr. Aliano:
On April 4, 2006 I inspected the galvanized side and window strapping for the house under
construction on Suffolk County tax parcel # 1000-83-1-15 in the hamlet of Cutchogue, New
York.
At the time of my inspection the house was sheathed and the windows and galvanized strapping
were installed. During my inspection I found both the side strapping (box beam to studding and
first floor studding to second floor studding) and the window strapping were properly installed in
an appropriate and satisfactory manner.

On March 16, 2006 I inspected the house under construction on Suffolk County tax parcel #
1000-83-1-15 in the hamlet of Cutchogue, New York for the purpose of checking the roof and
rafter strapping.
During my inspection I found that the rafter strapping had been completed and installed in a
satisfactory manner.

Re: (05-190) — Foundation damp proofing for new construction on tax parcel # 1000-83-1-15
Dear Mr. Aliano:
On February 9, 2006 I inspected the foundation damp proofing for the house under construction
on Suffolk County tax parcel # 1000-83-1-15 in the hamlet of Cutchogue, New York.
The damp proofing of the foundation for this lot was completed at the time of my inspection. It
consisted of a sprayed -on product known as "Mar -flex 5000" which appeared to be properly
applied in both quantity and location. The product has a 25 year warranty and seems to be more
than appropriate for use in this application.

Dear Mr. Aliano:
On April 27, 2006 I inspected the house under construction on Suffolk County tax parcel # 1000-
83-1-15 in the hamlet of Cutchogue, New York for the purpose of checking the insulation and
fire stopping of mechanical penetrations.
During my inspection I found that the insulation was installed in a satisfactory manner and that
the mechanical penetrations had been properly fire stopped.

1. Nailing requirements are based on wall sheathing nailed 6 inched on center at the panel edge.
If wall sheathing Is nailed 3 Inches on -center at the panel edge to obtain higher shear capacities,nailing
requirements for structural members shall be doubled, or alternate connectors, such as shear plates,
shall be used to maintain the load path.
2. When wall sheathing is continuous over connected members, the tabulated number of nails shall be
permitted to be reduced to 1-16d nail per foot.

§RR905.2.5 Fasteners. Fasteners for asphalt shingles shall be
galvanized steel, stainless steel, aluminum or copper rooting nails,
minimum 12 gage 10.105 inch (2.67 mm)] shank with a minimum 3/8 -Inch
(9.5 mm) diameter head, ASTM F 1667, of a length to penetrate through
the roofing materials and a minimum of 3/4 Inch (19.1 mm) into the roof
sheathing. Where the roof sheathing is less than 3/4 inch (19.1 mm)
thick, the fasteners shall penetrate through the sheathing. Fasteners
shall comply with ASTM F 1667.
§RR905.2.6 Attachment. Asphalt shingles shall have the minimum number
of fasteners required by the manufacturer. For normal application,
asphalt shingles shall be secured to the roof with not less than four
fasteners per strip shingle or two fasteners per Individual shingle.
Where the roof slope exceeds 20 units vertical In 12 units horizontal
(20:12), special methods of fastening are required.
EXCEPTION: Asphalt strip shingles shall have a minimum of six
fasteners per shingle where the roof Is in one of the following
categories:
1. The basic wind speed per Figure RR301.2(4) is 110 miles per
hour (177 km/h) or greater and the save is 20 feet (6096 mm) or
higher above grade.
2. The basic wind speed per Figure RR301.2(4) is 120 miles per hour
(193 km/h) or greater.
3. Special wind zones per Figure RR301.2(4).
